Weights/Conditioning / I will be unable to run my normal program this summer due to family obligations. / For a reasonable price, I can set up strength training and speed and agility with a qualified NC football coach who has offered his expertise. Let me know if you are interested in a package of 10 sessions.
Jr. Lady Warrior Camp / June 6-9
Monday – Wednesday 9am-2pm
Thursday
9am-12pm / All veteran players should plan to work camp for community service, all players new to program should plan to attend as much of camp as possible for reduced price of 25.00 to help cover salaries for staff. College players will be available to work with you along with our NC staff. / Report 15 minutes early each day and wear NC colors (veterans).
New players: Wear NC colors when you can @ this camp only.
All players will be dressed in team gear @ C. Classic & U of A.

AM – Fundamentals

PM/Evening – Camp events / This is going to be a fantastic softball experience for all of the NC players who attend. The University of Alabama is one of the premier NCAA teams in the nation. Patrick Murphy and his staff have won numerous coaches awards at the highest level. The NC players will get to work with most if not all of the Alabama players. One of our very own (she went to Kenn Mt, but I claim her too) Jennifer Fenton is a big part of Alabama’s success. Jen has consistently helped out at my camp and is very humble about her tremendous talent. Please make every effort to attend this camp as I know it will benefit our North Cobb Lady Warrior program. The camp will certainly be a wonderful experience for any softball player individually as well.
9. The team will eat in one of the college
Cafeterias. I have not received the exact
location.
10. The cost for the camp is 325.00. Please make
check payable to North Cobb Fastpitch Booster
Club.
11. A 175.00 down payment is due by June 1,
2011. The remaining 150.00 is due by July 11,
2011.
12. If an adult member of the player’s household is
able to help provide transportation to or from
team camp at the U of A, please let me know.
Even if we have already talked about your
commitment, please let me know again to
confirm. Provide number of players that you will
be able to transport. / 1.  Players are responsible for transportation to and from the University of Alabama, but we always try and provide transportation for those parents who cannot take off from work to help with the car pool.
2.  Each player attending must have a copy of their Cobb County physical turned in to me (Coach Weaver) so it can already be copies. The physical must be current.
3.  Each player must have her parent/guardian sign the Patrick Murphy registration/waiver form.
4.  Each player must provide proof of insurance by making a copy of her current insurance card.
5.  Players will room two girls per room.
6.  Players will stay in a University of Alabama Dormitory.
7.  Coaches and Chaperones will stay in the same dormitory
8.  Specific lodging for players or adults has not been given to me.
Essential Forms / Forms or items 1-4 must be turned into Coach Weaver to participate this summer!! Registration forms must be filled out and turned in for each camp that you attend. Players new to the program should make an effort to attend the Jr. Lady Warrior Camp for $25.00.

1.  Cobb County Physical Form – All sections must be filled out including the 2 page form

from a Medical Doctor or Physicians Assistant. Please double check that all signatures

of parent and player are signed.

2.  Waived or purchased insurance information on Cobb County Physical Form -

Information for purchasing school insurance can be found on our website at

www.northcobbfastpitch.org or on Cobb County School website under athletics and then

insurance.

3.  Information sheet – Update yearly (make sure all emails and cell phones are listed)

4.  Commitment Sheet – If you are filling this out for the first time, mark events that you

have already participated in and those that you will participate in. Make sure that one

parent or guardian and the player sign this form.

5.  Cherokee Classic Registration Form – Please fill out and pay $40.00

6.  University of Alabama Registration Form – Please fill out and pay down payment of

$175.00.

7.  Practice gear order form – Will be available at practice on May 31 and June 1.
