INSTRUCTIONS FOR COMPLETING FORM JASMA FORM F2

  1. Enter the one- or two-letter ICAO identifier as contained in the most current version of Doc 7910. If more than one identifier is designated for the State, use the letter identifier that appears first.

ICAO State Code for Japan: RJ

  1. Enter the operator’s three-letter ICAO identifier as contained in the most current version of Doc 8585. For general aviation aircraft, enter “IGA”. For military aircraft, enter “MIL”, and write the name of the operator/owner in the remarks row.
  1. Enter the one- or two- letter ICAO identifier as contained in the most current version of Doc 7910. If more than one identifier is designated for the State, use the letter identifier that appears first.
  1. Enter the ICAO designator as contained in the most current version of Doc 8643, e.g. for Airbus A320-211, enter A320; for Boeing B747-438 enter B744.
  1. Enter the aircraft series or the manufacturer’s customer designation, e.g. for Airbus A320-211 enter 211; for Boeing B747-438, enter 400 or 438.
  1. Enter the manufacturer’s serial number.
  1. Enter the registration mark of the aircraft, e.g. for AA-XYZ write AAXYZ.
  1. Enter the ICAO-allocated Mode S aircraft address code (6 characters, hexadecimal).

This Mode S address code begins with the proper bit string 84, 85, 86, 87 as defined by ICAO for Japanese-registered aircraft.

  1. Enter date in ddmmyy format, e.g. for September 12, 2016 write “120916”.
  1. Enter “Y” for yes or “N” for no.
