Tuition for the school year is due and payable upon registration; however, monthly payments are available to help make tuition more affordable for each family under the following provisions:

I/We the undersigned agree to the following tuition payment requirements for the 2017-2018 school year:

1.  A $200.00 non-refundable deposit fee, per new student, including any Pre-Kindergarten student, and a $75 non-refundable deposit fee for each returning student is required. These fees are to be paid with your pre-registration paperwork and will be deducted from your base tuition.

2.  FACTS Tuition Management Program~

Ø  All families are required to enroll in FACTS Tuition Management Program - online.factsmgt.com/signin/3VPLB. This site can also be found on the Holy Rosary Academy website; Families who already have a FACTS account set up with a high school will need to add Holy Rosary Academy by accessing the site listed above.

§  Annual (one payment) FACTS fee is $0. Payment is due 6/5/16.

§  Semiannual (two payments) FACTS fee is $10, per family, per school. The first payment is due 6/5/16 and the balance will be due 1/5/17.

§  The 10 or 12 Month Plan FACTS fee is $43, per family, per school. Payments start 6/5/16 on either plan. Monthly payments, both on the 10 and 12 month plan, will be due on the 5th of each month.

§  The FACTS fee ($10 or $43 – depending on plan option) will be automatically deducted from the account listed within fourteen (14) days of completed enrollment.

Ø  FACTS will assess a $30 NSF charge on all insufficient funds. Holy Rosary Academy will not assess NSF fees or late fees.

3. Any unresolved payments and/or late fees at report card time will result in report cards being held until payments are brought current. In addition, extra-curricular activities such as local/regional/national field trips and sports participation may be interrupted during delinquent periods.

4. Three late payments reported to us by FACTS may require the Responsible Party to pay the total tuition at the end of the month following the third violation. Non-resolution of delinquent payment(s) may result in student(s) inability to attend class until payments are current. Non-payment of the entire balance by the end of that month could result in dismissal of the student(s) from the school.

5. If a student is withdrawn for any reason (moving, expulsion, etc.), a refund and/or balance due of tuition shall be determined by the semester; one day into the semester, the entire semester amount will be due. In order to receive a refund of tuition paid up to that point (less pre-registration fees), the school must be notified no later than August 1 if a child is not returning to the Academy. Those enrolled the first day of school in the second semester are responsible for the entire amount of tuition for the second semester. Book and Art fees will not be refunded.

6. Upon withdrawal, the student's records will be held until all past-due payments are paid. This includes tuition, ESCAPE fees, library fees, lost book fees, Dining Hall charges, and late fees. In addition, a withdrawal form must be filled out and signed by the parent or guardian. Any outstanding balance due at the time of graduation will result in the student forfeiting his/her participation in all graduation events.

7. Report cards and interim report cards will be held if any payment is delinquent. This includes tuition, ESCAPE fees, Library fees, Dining Hall charges, field trip fees or non-returned sports uniforms and equipment. Access to student records will be held until all fees are paid. All bills are due at semester’s end.

8. Registration for the next school year will be suspended until the entire tuition account is paid.

9. Appeal of any violations of the above-stated provisions must be submitted in writing to the School Principal within ten (10) days of the occurrence. Appeals will be reviewed, and all decisions will be final.

Tuition Setup for New Families
Submit your pre-registration deposit of $200 per new student.
If applicable, bring the enclosed subsidy card to your parish office.
(They should return it to the school office as soon as possible.)
Enroll in FACTS Tuition Management Program.
1.  Go to online.factsmgt.com/signin/3VPLB and create an account.
2.  Choose Holy Rosary as your school.
3.  List the names of your student(s).
4.  Enroll in a payment plan (there are options for annual, semi-annual, or 10- or 12-month payment plans).
5.  Enter your bank account or credit card information.
6.  When you authorize the account on the last page, you’re set up!
Review and fill out the reverse of this form (Agreement for Tuition Payments). Return it to the school office along with the New Student Registration Form.
