Dear Parent or Guardian,
On the next pageyou will find the Parent Conference Evaluation Form.
There is a form for the fiction selections and a form for the non-fiction sections. You will find questions for the summer reading selections. Each question is meant to serve as a guideline for discussion of one of the summer reading selections.
Please have a conference with you student about one of the reading selections. The titles are listed on the forms. Your conference with your student should take approximately 15 – 30 minutes.
There are five areas for evaluation. For each group of two questions, points should be given based on your student’s verbal responses. A ten is the highest possible score for each group of two. The lowest score is a zero. A ten should indicate complete knowledge and understanding. Please total the points at the end of the conference.
Please print the forms before beginning. This evaluation must be signed by you and returned by your student to his or her tenth grade teacher of English no later than Wednesday, September 3, 2008. No evaluations will be accepted after this date. Your evaluation will be your student’s final grade for this book.
Keep in mind that this book conference will be a rewarding experience for both of you. Also, you may find this to be an opportunity to discuss other books and or experiences. Your cooperation is appreciated.
