Instructions

A curriculum change originates in the academic program’s school. The originator must complete and submit this form to the School’s Curriculum Committee. Once the School’s Curriculum Committee has approved this proposal and the Dean of the School has signed this form, the Dean then forwards it to the Administrative Assistant for the Office of Academic Affairs to be presented at the next College Curriculum Committee. After the College Curriculum Committee approves the proposal, the Provost for Academic Affairs forwards the proposal to the President’s Cabinet for review.

Notes:

·  The College Curriculum Committee will not review an incomplete form or one without signatures from the appropriate parties.
·  At least 1 additional document should be attached to this request form—the proposed curriculum/program-of-study.
·  This instruction page does not need to be included in the submission to the Committee.

Explanation of Terms:

Requested Term Effective / This is the term when the curriculum should be added to the Catalog. Typically, the catalog is published once per year in the Fall so the Term Effective would be the next Fall term.
