JOB TITLE:ADVANCED NURSE PRACTITIONER

JOB BAND:

Practice Manager for all other aspects of the job

Purpose of the Job

To provide medical services to patients presenting with acute needs, ensuring the highest standards of care, in accordance with the practice’s Personal Medical Services (PMS) contract.The role will also include health promotion, family planning and sexual health. The role holder should be able to prescribe and will be able to reach safe clinical decisions within professional boundaries on an autonomous basis.

Responsibilities

Clinical

  • Make professional, autonomous decisions in relation to presenting problems including

history taking, examination, investigation, diagnosis, treatment and referral where

appropriate, whether or not that patient has already been triaged by the practice. To follow

up such patients as is necessary.

  • To provide the above not only in 10 minute face to face consultations but also to undertake telephone consultations and home / residential home visits.
  • To manage acute exacerbations of patient’s chronic diseases.
  • Contribute to the development of and adhere to protocols and care plans for the

management of chronic disease and common medical conditions.

  • To provide a family planning / sexual health service,supporting and managing the health needs of women presenting for family planning, cervical cytology or sexual health consultation
  • Provide appropriate preventative healthcare, counselling, education and advice.
  • Prescribe and review, as necessary, in adherence to the practice’s prescribing formulary.
  • Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures and to maintain the utmost confidentiality in relation to such records.
  • Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care
  • Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment
  • Work in the context of a multidisciplinary team with a collaborative, supportive and

holistic approach, including referral to, and liaison with hospital services or other agencies.

Quality

  • Recognise and work within own competence and professional code of conduct as regulated by the NMC.
  • Prioritise, organise and manage own workload in a manner that maintains and promotes quality
  • Monitor work areas and practices to ensure they are safe and free from hazards and conform to health, safety and security legislation, policies, procedures and guidelines
  • Apply infection-control measures within the practice according to local and national guidelines
  • Contribute towards meeting the indicators outlined in the QOF and other locally and nationally agreed guidance.
  • To undertake audits to better understand outcomes and improve healthcare.
  • Participate in clinical governance activity and contribute to the improvement in

quality of health outcomes through the practice’s audit programme.

  • Use technology and appropriate software as an aid to management in planning, implementation and monitoring of care, presenting and communicating information

Education

  • Take responsibility for own learning and performance including participating in clinical supervision and acting as a positive role mode
  • Engage in appropriate educational activity in order to maintain clinical competence

and performance, and completion of objectives agreed by yourself and the practice in an

annual Personal Development Plan.

  • Provide an educational role to patients, carers, families and colleagues in an environment that facilitates learning
  • Participate in the training and development of staff in the practice.

Other

  • Support the practice’s admin team and respond to requests for advice and assistance.
  • Perform clinically related administrative and non-clinical duties necessary for the delivery of Personal Medical Services.
  • To participate in practice research activities.
  • Attend regular practice and clinical meetings as necessary.
  • Work as an effective and responsible team member, supporting others and exploring the mechanisms to develop new ways of working
  • Such other activities as the practice may specify

The above responsibilities will be undertaken upholding the following Best Practice Guidelines which all professionals working at Frome Valley Medical Centre are required to adhere to:

1.To be available to take over responsibility of patient care from the Out of Hours service, when so rostered, at 8am on a working day.

2.To continue to work until all clinical tasks have been completed. Where this involves

standing by ready to be called, to be available until the end of the session. To make

arrangements for others to cover when planning to leave before the end of the session.

3.To maintain the practice’s high clinical standards by using clinical governance audits

and other information to review patients’ results, prescribing, disease etc.

4.To undertake those daily tasks such as results, letters, reports, repeat prescriptions etc

arising from own consultations.

5.To maintain the financial security of the organisation by ensuring accurate and efficient

responses to income generation such as reports, dispensed prescriptions, private forms etc.

6.To undertake, as part of a team, those duties specified as being required.

7.To work as a team to cover requests for telephone contact.

8.To participate in the educational and organisational activities of the practice in a way that

maintains high standards and encourages the development of Frome Valley Medical Centre

as a “Learning Organisation”.

9.At all times to behave in a professional way that encourages quality practice and the

development of team spirit.
