Part Two: Budget and Statistical Narrative

CorePP-A.2 Budget Notes [Name of organization]
In addition to completing the CADAC financial forms online, organizations must also submit a page with the above label and respond to the following questions:
1) Indicate changes in the amount of the accumulated deficit OR surplus at the end of the organization’s last financial cycle, and projected for the current year, and/or request year (s).
a. Provide a brief explanation of significant variances (greater than 10% of total revenues) from one year to the next.
b. Comment on the current financial situation of your organization, indicating how it plans to use the accumulated surplus or manage the accumulated deficit, if applicable[1] in the year(s) of the grant request.
2) Provide a brief explanation of significant variances (greater than 15% from one year to the next) in other relevant line items in which they occur.
3) Briefly comment on the three revenue streams of (1) self-generated revenue, (2) government support, and (3) fundraising. Comment on your efforts to diversify revenue sources.
4) Submit any other budget notes that you feel may be relevant to the assessment of your application.
5) Submit any notes relative to the statistical form that you would like to provide for clarification or context.
Do not submit more than ONE PAGE for this section. Assessors are not required to read beyond one page.
Part Three: Activity Report

Assessment will be based on the merit of the organization and its programs* based on four areas of evaluation: A) Organizational health, B) Program excellence, C) Audience and outreach and D) Cultural sector contribution. Please describe recent, current and proposed activities under the headings provided below.
Your organization may not have the second or third year of programming fully in place. Please provide as much information as you are able – the Department understands programs in years two or three may be subject to change and that Program Officers will be updated on changes through the annual reporting mechanism.
*Programs – includes performances, exhibitions, productions, presentations and educational and outreach programming.
General and Contextual Information
Describe your organization taking into account the points below (We suggest limiting this section to one page or less). Highlight significant events that have contributed to the development of your organization.
· The mandate, mission and objectives of the organization;
· A brief history of the organization (date of establishment, major milestones and achievements);
· Any other relevant information related to its establishment; and
· The cultural context of your community and how your organization is positioned to meet the community’s needs (whether this be local, regional, national or international - this may be further elaborated in the narrative below).
A. Organizational Health: Briefly describe the overall management and financial health of the organization. (We suggest limiting this section to one page).
Examples of the kinds of information you may wish to provide:
· A description of your organization’s strengths and challenges as they relate to its mandate and role within the community.
· A description of your human resources and any successes or challenges faced in this area, such as recognition, staff turnover, remuneration, mitigation of risk etc.;
· Strategic partnerships your organization undertakes that help it achieve its goals;
· Professional development opportunities or mentoring provided to staff and/or board;
· The organization’s strategic planning process and how it evaluates its own effectiveness.
B. Program Excellence: Please elaborate on the artistic performance and merit of your organization and how your work contributes to the arts discipline. Describe current and future programming as far as it is planned. (We suggest three to four pages for this section).
Do not list your programs in this section - A list of previous and planned programs is required in Appendix 1.
Examples of the kinds of information you may wish to provide:
· A description of your organization’s artistic vision and how its programming furthers that vision;
· A description of your organization’s programming focus for the current and coming years, highlighting milestone programs or events;
· Evidence of how your organization provides fair and appropriate fees/remuneration for its performers/artists;
· Mention of awards or prizes, positive critiques or public recognition your organization has received in the past year, only.
· A description of how your organization strives to grow and evolve in artistic content, programs and excellence.
· A description of how your organization demonstrates artistic leadership and mentorship in its community.
C. Audience and Outreach: Please describe your outreach and audience development efforts and achievements. (We suggest one to two pages for this section).
Examples of the kinds of information you may wish to provide:
· A description of the types of audiences your programs reach.
· A description of your organization’s contribution to the education and/or social development of your audiences or your community;
· A description how your programs contribute to better understanding of the artistic discipline or subject matter;
· How your organization evaluates its audience and plans to grow or diversity its audience through research and evaluation;
· Efforts that have been made to reach under-served segments of society;
· A description of partnerships undertaken with community organizations to reach the organization’s outreach goals;
· Recent exporting or touring activities.
D. Cultural Sector Development: Please elaborate on the contribution of the organization to the development of the cultural sector and to the cultural infrastructure of the province. (We suggest limiting this section to one page or less).
· Using New Brunswick’s cultural policy Creative Futures as a guide, indicate which of the policy’s goals and objectives that your organization most advances, or intends to advance with your activities. Please limit your description to a maximum five goals and/or objectives.

· The current year (2015-2016)
· Year 1 of the multi-year cycle (2016-2017)
· Year 2 of the multi-year cycle (2017-2018)* as currently planned
· Year 3 of the multi-year cycle (2018-2019)* as currently planned
Include if appropriate: Name and size of venue, and total audience per event.
*It is recognized plans for Years 2 or 3 may be subject to change. Organizations will provide program updates in their annual reports to the province.
Addendum 3: Audited Financial Statement and Annual Report (required)

Addenda 4 to 6: (Optional)
Please number and label the addenda as described above. Only Addenda #1 - #6 will be forwarded to the assessors who are only required to review up to 4 pages of each addendum. All addenda are required to be submitted electronically. (Please see guidelines below for optional submission of hard copy support material).
Examples of types of addenda include:
· A recently completed strategic plan (Executive Summary preferred). Comment on your organization’s performance against its strategic plan in your main narrative.
· A compilation of reviews, articles or program recognition, from print, web or other media for the past year only (scanned or pasted maximum 4 pages).
· A marketing plan (Executive Summary preferred, maximum 4 pages).
· A page of web links (maximum 3 links) that relate to your organization’s programs.
· Other (please specify)

[1] Organizations are not penalized for accumulated surpluses provided there is a sound rationalization for their use (i.e., they would be carried forward for a future project, maintained as a contingency fund against future expenditures, invested into an endowment, etc.) All accumulated deficits must be addressed in the narrative. Deficits greater than 15% must be accompanied by a deficit reduction plan.
