Dance Guest Pass Instructions
1. Fill out entire form completely and clearly (please print neatly on all lines with the exception of where it states a signature is needed).
2. Have guest obtain signature/permission from his/her school (if guest is not in school please attach a copy of their Driver’s License)
3. Turn form into your Assistance Principal for approval
4. Once approved pick up form and purchase ticket
