ADMISSION AGREEMENT

Note: Regulation Section 101219 requires that facilities that care for preschool aged children maintain a current written admission agreement for each child attending.

  1. This written agreement for care is betweenEl DescansoElementary Preschool and ______(parent/guardian) for the care of ______(child’s name).
  1. The parent or guardian agrees that s/he is contracting for the services which are check marked below and will pay the stipulated rate.
  1. Care and Supervision

a. Tuition

AM Preschool $325.00 per month ______

M-F (8:15-11:15 a.m.) PVNfL Scholarship ______

 PM Preschool $325.00 per month ______

M-F (12:00- 3:00 p.m.) PVNfL Scholarship ______

All fees are due and payable on the first Monday of the month prior to care being received. Payments will be considered late if not paid by Wednesday following the first day of the month. Fees are not prorated or refunded due to a child’s absence. Fees are averaged based upon the annual cost and will not be prorated for vacations or school closures.

b. Snack

AM Snack –Parents are responsible for the cost of snack or providing a snack from home. Free and low cost meals are available to those who qualify.

PM Snack- Parents are responsible for the cost of snack or providing a snack from home. Free and low cost meals are available to those who qualify.

  1. The parent/guardian is aware that the State of California Licensing Agency (Community Care Licensing) has the following authority:
  1. To interview children, or staff, and to inspect and/or audit child or facility records without prior consent.
  2. To observe the physical condition of children, including conditions which could indicate abuse, neglect, or inappropriate placement, and to have a licensed medical professional physically examine the child(ren).
  1. The parent/guardian agrees that s/he has received and read a copy of the Parent Handbook and agrees to all the Preschool’s operating policies and procedures therein. Please note that all children must be toilet trained and 4 years old by December 2.
  2. The agreement may be terminated by the Preschool with 4 weeks notice for the following reasons:
  3. Parent/guardian has not paid fee at least twice during six months
  4. Child is not attending preschool regularly.
