Instructions for Request to Close action in KC

Once research has been completed, the KC system will need to be used to close the protocol and a final report will need to be completed. The final report form and instructions can be found using the following links.

Please save the completed form on the machine that will be used to complete the “Request to Close” action in KC because it will be necessary to upload and attach the form directly in KC.

Step 1: Find the protocol you would like to request to close.

1.  Click on the “Search Protocols” link in the IRB Protocols box of the Researcher landing page (upper box, middle column.

2.  Using the Protocol # field, type your protocol number in the format *2#####* in the box.

  1. If you do not know your protocol number, you can use any of the other fields such as Title or Investigator (PI).

b.  Always remember to use * character before and after your search criteria.

3.  Hit the enter key or click the “Search” button.

4.  Scroll down to see your results.

5.  Select “Edit”

Step 2: Perform the Request to Close Action.

1.  Click the “Protocol Actions” tab.

2.  Show the “Request an Action” folder.

3.  One of the available actions will be “Request to Close”

4.  Click “Show” to the left of “Request to Close”.

5.  Select the Committee. Choose the committee to which the protocol was originally submitted.

6.  Type a reason for the request (e.g. “research concluded”)

7.  Click the Browse button to find the final report form.

8.  Type a description of the file in the description box. (e.g. “Final Report form”)

9.  VERY IMPORTANT. Click the ADD button on the far right to upload the file to KC.

10.  Click the Submit button.

This is what the action should look like just prior to clicking the “submit” button. (In this example, the SBE committee was chosen. Choose the committee to which the protocol was originally submitted.)

Once submit has been clicked, inform the correct committee administrator (either or ) via email.
