- YoumustcompleteanEnhancedDisclosureandBarringServiceapplicationformif a Disclosure and Barring Service (DBS) Check is required for your research and you do not have an existing DBS check that has been carried out in the last year.
- If you have had a DBS was carried out in the last year by the NHS Trust where you will be carrying the research out, please check with the Criminal Records Officer (details at the end of this form), whether the DBS falls into the correct category (Enhanced Disclosure). If your DBS does not fall into the correct category, you will need to have a further DBS check carried out by Anglia Ruskin University (please complete the DBS Continuous Consent Form for Student Researchers Applying for a DBS Check via Anglia Ruskin University).
- If the DBS you have already had done does fall into the correct category, you will need to show theCriminal Records Officer the original version of your DBS clearance. Upon receipt of a satisfactory Enhanced Certificate, the Criminal Records Officer will then notify the Head of UK/EU Admissions of the date of the DBS Certificatewhich will be included on the NHS Passport application.
- Alternatively, if you have previously subscribed to the DBS OnlineUpdateService,you need to notify the Criminal Records Officer of this and providecontinuousconsent (by signing this form) and the original DBS DisclosurecertificatetotheCriminalRecordsOfficer. The Criminal Records Office will then check the details online and inform the Head of UK/EU Admissions that you have had a satisfactory DBS Certificateand add this into the NHS Passport application.
- You are not permitted to start your research until youroriginalDBSEnhancedDisclosurecertificatehas been viewed and verified bytheCriminal Records Officer or theCriminal Records Administrator.
- Students whose DBS Disclosure reveals reprimands, cautions, convictions or additional information will be requiredtosubmitfurtherinformationasrequestedbytheCriminalRecordsOfficerforpresentation tothe Disclosure Paneland willnotbepermittedtoattend placementsunless permission is received from the Disclosure Panel.
