Organisational Background

The South Eastern Consortium Alcohol and Drug Agencies (SECADA) formed in 2014 to provide alcohol and other drug (AOD) services in the South Eastern Catchment. The consortium comprises South Eastern Melbourne Medicare Local as the lead agency in collaboration with Odyssey House Victoria, Taskforce Community Agency, YSAS Pty Ltd, Windana Drug & Alcohol Recovery and Monash Health, connected through an MOU and a united commitment to providing opportunities for change and growth in the catchment by reducing AOD use, improving physical and mental health, and reconnecting people to their families and the community.

The Intake and Assessment Worker will report directly to the Windana Team Leader of Community Services while participating in training and face-to-face team meetings as part of the SECADA Intake and Assessment Team.

Established 1984 Windana is an independent organisation, incorporated under the Associations Incorporation (Amendment) Act 1997 and managed by a Board of Management.

Windana is a major provider of drug treatment services in Victoria. Our aim is to offer holistic, innovative services through encouraging our people and clients to work in partnership. Organisation Context:

The SECADA intake and assessment service is staffed by workers at hubs throughout the catchment.

The intake and assessment worker will collaborate closely with other related services, including mental health, primary care, community health, Aboriginal and Torres Strait Islander health and migrant health services to support appropriate referrals to and from the AOD services. They will also offer a suite of self-directed care options for people who do not want or need face-to-face treatment.

The Windana Intake and Assessment Worker for SECADA will work from a Dandenong site with requirement to work across the catchment as needed.

Objective of the Position:

Directly accountable to the Team Leader of Community Services, the Intake and Assessment Worker is responsible to:

·  Provide Intake and assessment (I & A) as part of the SECADA I & A Team.

·  Develop and maintain effective relationships with sector services to ensure cooperation in the overall delivery of services to clients and their families.

·  Improved coordination and monitoring of complex clients and clients entering specialist programs e.g. Slow Stream Pharmacotherapy.

·  Work within an interdisciplinary team to embed a culture of ‘one person, one plan’ model of care.

·  Assist to reduce the dependency on alcohol or other drugs within a harm minimisation framework.

·  Work with an interdisciplinary assessment team to provide care coordination for clients accessing Windana.

·  Improved access to timely treatment.

·  Develop and maintain excellent communication and relationships with services.

·  Improve pathway and seamless interface between internal consortium and external programs

Liaise with (key internal and external contacts):

SECADA staff, community, Allied Health professionals, AOD consortia and other agencies.

Responsibility

·  Work within the parameters of general legislative compliance – including but not limited to OHS laws and guidelines, Fair Work Australia Act, Children and Young Persons Act, Privacy Act, Health Records Act

·  To abide by all guidelines and requirements of our principle funder (s)

·  Follow the objectives of the service plan / model as directed by your Team Leader / Manager

Position responsibilities

·  Provide a service at the initial point of contact for the central intake system and deliver an efficient and comprehensive screening and assessment service for individuals requesting access to services

·  Ensure an efficient and coordinated approach to service delivery and referral

·  Facilitate the delivery of high standards of coordinated care

·  Develop strong relationships with the Training and Support officer and partner agencies and provide regular progress reports to all relevant stakeholders

·  Ensure accurate recording, maintenance and security of any client records

·  Client specific ITP’s on each client’s file and evidence of working with client according to ITP. Clients confirm consultation and input into ITPs

·  Clients care plans, files notes and supervision review

·  Attendance and cooperation at internal and external meetings

Selection criteria

Mandatory Skills & Competencies:

Key selection criteria:

·  Preferred: Tertiary qualification, such as Social Work, Allied Health, Counselling or Community Services.

·  Certificate IV in AOD qualification (completed, commenced or willing to commence within the probationary period).

·  Experience working within Alcohol and Other Drug and/or mental health treatment settings.

·  The ability to take a client centred approach to prioritising needs and providing information, intervention and referral.

·  Assessment skills.

Qualifications:

·  AOD Certificate IV

·  Bachelor degree or higher qualification in a relevant field is highly desirable

Employment Conditions

Salary and conditions are in accordance with the Windana Drug and Alcohol Recovery Inc. Enterprise Agreement 2016. This position is full time – 76 hours a fortnight.

Remuneration Packaging is available in accordance with current legislation.

All positions at Windana are subject to on-going government funding.

The Intake and Assessment Worker is expected to conduct her/himself with professional integrity towards clients according to the “Staff Code of Ethics” and maintain their professional responsibilities in line with the “Bill of Rights for Members and Clients”.

Under the Accident Compensation Act, it is the applicant’s duty to advise Windana of any pre-existing condition, which could be aggravated by the type of employment they are applying for. Failure to do so seriously jeopardises any entitlement the employee might have for a work-related aggravation of that non-disclosed pre-existing condition.

Agency vehicles are available for authorized use and these should be used at all times for work-related purposes.

Other employment conditions are as per the Award / Agreement.
