Overall GPA______Current Grade Level (8, 9, 10, 11)______

Your current English teacher, including first name if you know it ______

1. What overall grade did you receive in your English class this past semester?______

If you received less than an A or B, please explain why.

2. List two or three of your favorite movies, plus a brief description of why you like each film.

3. Do you ever read books for pleasure, other than required reading for school?

4. If you answered "yes" to Question #3, name two or three books you've read for pleasure that you've really liked, and tell me a few things you liked about them.

5. Please attach—preferably by stapling—a typed page of some of your best creative writing.

Please provide one page, double-spaced, approximately 200 to 250 words in length. This creative writing sample may be from a short story you’ve written, or from a personal essay. Please do not include an excerpt from a research paper, such as a paper you would write for a social studies class.

Your creative writing sample does not need to be longer than one page. You will not be penalized for providing more than one page—however, I may not read more than your first page, so please make sure that the first page is some of your best creative writing. Instead of an excerpt from a short story or personal essay, you may include poetry or a script (either for stage or screen); however, if you include a poem or script, make sure it is a stronger example of your creative writing than anything else you’ve written. My preference is for a sample from a short story or creative personal essay.
