APPLICATION FOR CERTIFICATE IN INJURY CONTROL

Students currently enrolled in a masters or doctoral degree program at the Johns Hopkins Bloomberg School of Public Health or non-degree students who have a graduate degree from an accredited school of public health can apply for the certificate in Injury Control. Requirements for the certificate are listed on the second page of this application and include completion of: (1) three required core courses and two one-credit seminars (10 credits); (2) one course focusing on a specific injury problem; (3) and two courses related to common injury control strategies. All courses taken to fulfill certificate requirements except for the seminars must be taken for a letter grade. Certificate students must ha 3.3 GPA in courses leading to the certificate. Course substitution is only permitted by authorization of the certificate program director.

Students who would like to receive the certificate should complete this entire application form (2 pages). The completed application form and a recent copy of the student’s School of Public Health transcript should be submitted to the Center for Injury Research and Policy, (Dr. Daniel Webster, Hampton House Room 593) no later than the first week of the term in which the final course leading to the certificate is offered.

Incomplete applications will not be processed. Notation of certificate completion will appear on the student’s transcript. The Injury Control Certificate will be mailed after grades have been posted, approximately 4-6 weeks after the end of the term.
