Western Suffolk Counselors’ Association Graduate Award

2011-2012

APPLICANT: To be considered for the WSCA Graduate Award, please complete this application form and attach the following documents:

1.  A student copy of your graduate school transcript

2.  Two letters of recommendation. One must be from a faculty member in your counseling program and the other should be from another counseling professional who can comment on your abilities, (preferably an internship supervisor or perhaps someone you worked with professionally in a counseling capacity i.e.: Leave Replacement position etc…)

Letters should arrive in sealed envelopes

3.  Typed Personal Statement. In approximately 350 words please include your opinion on the importance of both your present involvement in Western Suffolk Counselors’ Association as well as your future professional commitment to counseling associations.

4.  A resume

5.  Provide documentation on one of the following:

·  Residency in Suffolk County (i.e., copy of driver’s license)

Or

·  That your graduate school is located in Suffolk County

Or

·  That your internship is taking place in Suffolk County (please give us the name of your Internship Supervisor at the school you are working in)

Compile all materials and mail in one package to:

Winner will be notified at the end of May and will attend WSCA’S Annual Awards Ceremony

on Thursday, June 7, 2012 at the VIEW Restaurant in Oakdale, NY

**No further documentation is necessary if proof is provided in transcripts, letters of recommendation, etc….

to a deserving graduate student

Criteria for the Award:

1.  Applicant must be a full or part-time graduate student in a Master’s Degree Counseling Program.

2.  Must have completed a minimum of one year in this program or 12 graduate credits.

3.  Demonstrate high academic achievement, commitment and involvement in the counseling profession.

4.  Two letters of recommendation (one from internship/work supervisor-if applicable and one from a Graduate Counseling faculty member).

5.  Be a member of Western Suffolk Counselors’ Association (before December 31, 2011).

6.  A Personal statement on the importance of your involvement in Western Suffolk Counselors’ Association as well as your future professional commitment to counseling associations.

7.  Attend Graduate school, live, or intern/work in a school in Suffolk County.
