Take notice that the support obligation in this case will be enforced as set out in the attached orders.
You the OBLIGOR are hereby ordered that all future support payments in this case must be made through the HAMILTON COUNTY CHILD SUPPORT SERVICES. Payments made directly to the obligee (person who is receiving support) will be considered as gifts. Further it is your obligation to notify Child Support Services in writing, of any change in the status of the child for whom support has been ordered that would necessitate a termination of the support and withholding orders.
Any withholding order issued by this Court shall apply to all subsequent employers and should new withholding orders be necessary, they may include an amount to pay of any arrearage, up to a maximum amount permitted to be withheld under Section 303 (b) of the “Consumer Credit Protection Act” 15 USC 1673 (b).
Pursuant to the Ohio Revised Code S3113.21 you must immediately notify this Court in writing of any changes in the above circumstances and of any additional sources of income that you may obtain in the future. The notice regarding employment, including self-employment shall contain a description of the nature of any new employment and the name and address of any new employer. The notice regarding funds on deposit shall include any changes in the status of the account number and the name and business address of that financial institution.
If support is being withheld from the benefits or funds other than those derived directly from employment, or if a bond is being held by the Court to assure payment of support and you become employed, you must request the court to cancel that withholding deduction or bond and begin withholding from your personal earnings at your new employment.
If you are unemployed, you must regularly seek employment and immediately upon obtaining employment notify this Court as directed above. You must also immediately notify this Court in writing, upon obtaining any previously undisclosed income, or upon obtaining ownership of any asset with a value of $500.00 or more.
Should you default in making your support payments as ordered and have legal claim against another person of more than $1,000.00, you must immediately notify the Child Support Enforcement Agency in writing, of the amount and nature of the claim and of the name and address of the person against whom the claim exists.
Remember you must notify the Child Support Services of all changes.
All notifications should include your name, mailing address, social security number, SETS and Court case number.
