Job Title: Client Advocate/Outreach Worker
Immediate Supervisor: Migrant Health Program Manager
Essential Duties and Responsibilities: Provide quality services to Migrant Health users ensuring compliance with Federal and State guidelines; educate clients on health issues, provide support for medical provider; promote access of migrant/seasonal farm workers to medical services.
Major Duties:
1. Perform nursing skills, including, weight, and height, blood pressure, and lab procedures.
2. Conduct intake interviews, prepare rooms, and assist provider as needed; document clinic
activities
3. Counsel client, provide information and appropriate referrals and follow-up as required;
conduct post counseling and documentation.
4. Provide general clerical duties, input data, complete statistical records, audit and monitor
vouchers, prepare client charts; stock and maintain an inventory clinicsupplies and
pamphlets.
5. Promote Migrant Health and facilitate accessibility for migrant and seasonal farm
workers, which may include transportation and translating.
6. Follow up missed appointments, completion of medical regimens, and etc.
7. Perform maintenance functions as needed for health clinic, suitcase and satellite clinics.
8. Maintain an understanding of and operate under guidelines of Nebraska Department of
Health and agency policies and procedures; work cooperatively to meet program
performance standards, workplace goals and client goals.
9. Submit reports as requested.
10. Attend staff meetings and training opportunities as required.
11. Participate in the Performance Measurement and Improvement (PMI) activities.
12. Maintain patient confidentiality
13. Provide interpretation for Spanish speaking clients
14. Perform other duties assigned.
Minimum Qualifications:
Grade 11:
1. High School diploma or equivalent with two years experience in a health field.
2. Written and oral communication skills in English and Spanish.
3. Must be able to complete the Medication Aide class and maintain licensure.
4. Ability to maintain a current CPR certification
5. Must be able to work irregular hours and evenings.
6. Computer experience preferred.
7. Age 21 or above.
8. Valid driver’s license with good driving record and insured vehicle available.
9. Physically and mentally able to perform duties.
Grade12:
1. Must meet the qualifications above.
2. Medication Aide with valid Nebraska license and one year experience.
Grade 13:
1. Must meet qualifications above
2. Additional training specific to this position.
Work Environment: The noise level in the health clinic is usually elevated. The workplace environment characteristics, included in this job description, are representative of the conditions an employee will encounter while performing the essential functions of these duties.
