UPWARD BOUND CONTRACT
2013—2014
I, ______, as an Upward Bound participant, expect to receive the
following from the program:
1. Instruction in math, English, social studies, science foreign language, and computer.
2. Tutoring as needed in math, English, social studies, science and foreign language.
3. Assistance with the AHSGE, ACT and SAT Prep.
4. Academic and postsecondary Counseling.
5. Career Counseling and Career Mentoring.
6. Personal Exploration Opportunities.
7. Cultural enrichment and community service opportunities.
8. Mentoring activities.
9. Stipends during the year reflecting $10.00 per session attended will be received by
permanent roster students who have met the program’s requirements. Newly admitted
students will be eligible for stipends beginning in January if program requirements
have been met.
In order to receive the above benefits from the Upward Bound Program, I will adhere to the
following rules and regulations:
1. Attend at least twelve of the sixteen for the returning students and fifteen for new students
evening sessions and/or Super Saturday make-up sessions.
2. Maintain a minimum grade point average of 2.5.
3. Meet testing requirements
4. Maintain acceptable behavior in my home high school and while I am with the program
at the University of Montevallo.
5. Attend at least 3 cultural enrichment activities each semester and 1 community service
project per semester.
6. Attend tutoring sessions as needed and/or required.
7. Return absentee forms explaining any absences from evening sessions.
8. Participate in fundraising efforts or forfeit scholarship opportunities through the
Upward Bound Foundation Fund.
Should I fail to follow all of the rules and regulations, I expect the Upward Bound program to take
action that will hopefully correct the problem. This action might be in the form of one or more of the
following: 1. A parent conference.
2. Probation.
3. Dismissal from the Upward Bound Program.
The terms of the Upward Bound contract have been explained to me and I agree to adhere to
these terms. I will also receive a copy of this document.
