Form of Consent for the Application to the JST-CREST/PRESTO/ACT-I Programs

This form must be filled by a person in charge of collaborative research agreement at a research institution located outside Japan and submitted to Japan Science and Technology Agency (JST)if a researcher affiliated to the same institution will be a Lead Joint Researcher of a CREST research proposal or anApplicant of a PRESTO or ACT-I research proposal.

If the research team will include overseas research institutions/If the applicant of PRESTO or ACT-I proposals will pursue the research project at an overseas institution, please confirm that the overseas institution fulfills the requirements below, due to contractual reasons (as a general rule, no contract will be concluded if these requirements cannot be accepted). Please review the requirements below, and take the appropriatemeasures to provide a prior explanation and receive prior agreement from the overseas institution.If no “agreement” is acquired at the time of submission of this “Supplemental Document,” it is possiblethat changes to the research plan may be requested on the day of the interview or at the time the applicant becomes a candidate for selection.
