APPLICATION FORM FOR STUDENTS TO BE DRIVEN BY OTHER STUDENTS

Dear Parent/Caregiver

Please complete and return this permission form if you wish your son/daughter to be driven to and from EOTC activities by other students. In signing, you acknowledge that there are inherent dangers and risks in students’ use of vehicles that may lead to injury or death and you acknowledge that it is your responsibility to check that student drivers have the required licence and that vehicles in use are roadworthy, registered and have a current WOF and that the driver is a person you would be happy with as a responsible driver. Staff will brief students on behaviour expectations and will take all reasonable precautions to ensure safe travel.

I have read the accompanying information sheet regarding student vehicle use and passengers in vehicles driven by students and understand the risks associated with this and my responsibilities as outlined above. I acknowledge that students driving in an unsafe manner will have their permission to drive status to and from EOTC activities removed.

Please follow the questions in order and sign and date the parent caregiver section at the end of this document.

6I have read the accompanying information sheet regarding student vehicle use and passengers in vehicles driven by

students and understand the risks associated with this and my responsibilities as outlined above. I acknowledge that

students driving in an unsafe manner will have their permission to drive status to and from EOTC activities removed and

that my son/daughter would therefore be unable to travel as a passenger. I acknowledge that my son/daughter can only

travel as a passenger once the school has received and approved an accompanying form from the driver’s

parent/caregiver.
