6th Grade Disney Trip ~ November 16th - 18th
Medication Guidelines
1. Any medication that will be taken during the course of the field trip must be administered by a trained staff member, with the exception of inhalers and Epi Pens. (Inhalers & Epi Pens should be with the student at all times) Do not pack your child’s medication in their luggage, this is against school policy.
2. All medications must have a completed HRS 29-C or Short Term Medication Form. The forms are bright blue. If you listed medications on your student’s permission form, you will receive forms with this letter. Otherwise, you can request forms from the med tech or front office.
3. All medications Must be in their Original container. If your child takes a prescription medication and you only have one bottle, you can request an empty, labeled bottle from your pharmacy.
4. All medications and blue forms should be delivered to the clinic No Later Than - Friday, November 11th. Please call the med tech, Dayna Perkins at (478)988-6171 ext. 2, if you have any questions.

Remember - You will be responsible for administering the medication
required for the morning of departure.
