Select Your Club

Double click on the bar where the club name appears for your Region. A Drop Down Form Field will appear. This will bring up a window that lists all of the clubs in your region. Highlight your Club Name then use the up/down arrows to the right where it says “Move.” Move your Club Name to the top of the list. Once your club is at the top of the list, click OK and your club name will appear on the form.

CLUB CATEGORY: (Club Secretary)

Category B 21 – 35 members

Category C 36 or more members

Double click on the bar and a Drop Down Form Field will appear. This will bring up a window that lists the entire Category’s. Highlight in your Drop Down List your Club Category either “A,” “B,” or “C.” Then use the up/down arrows to the right where it says “Move.” Move your Category to the top of the list. Once your Category is at the top of the list, click OK and your Category will appear on the form.

Your club category is based on your last June 30th, 2015 WMMR report. This will be your category for the entire reporting year regardless of your actual current month membership. So it will remain the same all Lions year. So put it in once and leave it with no more changes this Lion Year.

Double click on the Month bar and a Drop Down Form Field will appear. This will bring up a window that lists all the Months. Highlight in your Drop Down List the current Month you are working in. Then use the up/down arrows to the right where it says “Move.” Move your Month to the top of the list. Once your Month is at the top of the list, click OK and your Month will appear on the form. Then Double click on the Year bar and a Drop Down Form Field will appear. This will bring up a window that lists the Years. Highlight in your Drop Down List the current Year you are working in. Then use the up/down arrows to the right where it says “Move.” Move your Year to the top of the list. Once your Month is at the top of the list, click OK and your Month will appear on the form. Remember the year changes with the Calendar Year not the Lion Year.

All issues of the newsletter must be mailed or emailed to the District 4-L5 Public Relations Chairperson, District Governor, Vice-District Governor’s, Cabinet Secretary, Region, Zone Chairpersons, Lions Pride Editor and Special Award Chairperson.

In the Default value two circles will appear, “Not Checked” or “Checked.” Click on either the “Not Checked” circle or “Checked” circle depending on what you want done either “Yes” or “No” then click OK. A box will then be checked with an X checked or it will be blank. Credit is given only once per month for only one bulletin. No credit given for second or more bulletins’ issued each month.

Enter the number of Official Club Visitations, Club # (Club Number can be found in the District Directory following the club name), Date Visited and # of Members Attending each visit made during the reporting month. Example of club #: ANZA #32193. (Each club has their own number assigned by LCI.) Points will be awarded per visitation – 10% of club membership must have attended.

Enter the number of Un-Official Club Visits, # of Club Visited, Date of Visit and # of Member(s) Attending each Visit. If visiting a club within the District 4-L5 Lions Clubs directory, the number follows the club name. Example: ANZA #32193. If the visit is outside District 4-L5 asks the club secretary or a member who knows what the Club # is. Use this section for individual club members that visit other clubs that are not official visits. Points will be awarded for individual Lions, not for club visitation credit per visitation.

Visitations must conform to the rules of the District 4-L5 Interclub Visitation Contest as outlined in the District Directory page 93. Do not report more than one visitation to the same club for each category of visitation award during the contest year for the Interclub Visitation Contest. Use the District Directory to find the club number. It is located to the right of the club name in the directory. If the visit is outside District 4-L5 asks the club secretary or a member who knows what the Club # is.

Credit will not be given unless the Lions Club specifically mentioned. Send Copies of Public Relations Articles to: Public Relations and Special Awards Chairpersons.

Double click on the “Yes” or “No” box. A Drop Down Form Field will appear. In the Default value two circles will appear, “Not Checked” or “Checked.” Click on either the “Not Checked” circle or “Checked” circle depending on what you want done and click OK. A box will then be checked with an X or it will be blank. If there is an article an X should be in the Yes box. If no article submitted this month check the “No” accordingly.

Attach copies of newspaper articles, pictures etc. Complete with Periodical name and date. Club name, and/or Lions member name in articles shall be highlighted. If you have received radio or TV coverage include a copy of the contract with the station. If PSA time is given provide a copy of the “Air Log” listing the ‘spots’ run. Credit will not be given unless the Lions Club is specifically mentioned or the Club’s Emblems or insignia are clearly visible.
