FUNCTION:Initiates home care referrals over the phone from physician offices, facilities, community agencies, and community at large by evaluating and arranging for services, supplies and equipment for home care. Serves as a resource for community services if home care referral is not appropriate.
REPORTS TO:Intake Manager
RELATIONSHIPS:Patients, families, caregivers, physicians and office staff, community, URMHC Home Care Coordinators,vendors, contract and regulatory community agencies, third party payers, UR Medicine Home Care departments and staff.
HOURS:40 hours/week

RESPONSIBILITIES:
- Assesses and evaluates the needs of referred patients through phone interview and patient chart review, identifying appropriateness for home care including an assessment of the safety, support system, identified caregivers to learn/perform care, patient willingness/ability to perform care, and payer/financial issues. Referrals are complete, accurate, and with timely communication to the Care Team Coordinators for scheduling.
- Possesses and utilizes knowledge of agency and community resources to make appropriate referrals for services, supplies, and equipment and arrange for timely delivery of such.
- Communicates and coordinates referral process with the interdisciplinary team at UR Medicine Home Care and at institutions, as well as with patients, including documentation per institution and UR Medicine Home Care standards.
- Actively participates in marketing UR Medicine Home Care services to referral sources ongoing.
- Initiates assistance of UR Medicine Home Care patient financial services for patients without insurance.
- Utilizes UR Medicine Home Care consultants to assure maximum efficiency in care delivery through use of new/improved technology and products.
- Maintains own productivity in answering calls and generating referrals as well as assisting others as needed to limit use of overtime and supplemental help.
- Responsible for the Key Performance Indicators (KPI’s) established by manager.
- Complies with agency and regulatory standards (HIPAA, OSHA, confidentiality), maintains attendance within UR Medicine Home Care standards, and completes required/mandatory in-services and professional licensing.
- Other duties or projects as assigned by manager.
QUALIFICATIONS:
- NYS licensed and currently Registered Professional Nurse.
- Three years of health care/community health experience preferred.
- Outstanding interpersonal and communication skills both verbal and written.
- Strong analytical skills to detect and analyze trends.
- Minimum two years electronic data and information management required.
- Call center experience a plus.
