Adult Carer Support

Our Adult Carer Support services are there to empower unpaid carers. Our expert team provides a wide range of practical and emotional support enabling individuals to care for those they love whilst maintaining a life of their own.

You are anunpaid carerif you support a family member or friend who is ill, disabled, elderly, has mental health issues or substance misuse problems. The support you offer may be emotional or include shopping, cooking, dealing with medication, or attending medical appointments with the person you care for. The person you care for could live with you, in their own home, or even in residential care.

Most of our services are for carers living in the city of Nottingham however we do have some services which are available for carer’s living within the county.

To register with us please contact us using any of the above methods.

If you or a carer you know have any specific information or communication needs and require information about our services in an alternative format please let us know.

Counselling

Carers Federation counselling support was established in 1996 as part of a developing service to carers. We provide emotional support to reduce the symptoms of anxiety and non-delusional depression, and for carers to realise their individual potential thus improving the quality of social and interpersonal relationships. For carers, a major outcome has been defined as prevention of carer role breakdown.

How we do it - Our counsellors recognise the struggles, joys and responsibilities that the carer can encounter within their caring situation. The feedback from clients is that the counselling has helped to reaffirm their sense of worth and self-identity, thus empowering the client in their given situation.

How we deliver it - The service offers:

Counselling for Carers Aged 7 to 80+

Support in venues that suit you. These include: Our city centre rooms, at local suitable premises,

at GP practices and at schools

Home visits for adults can be arranged if assessed as suitable.

Cost – There is a small cost for people living within the county of £6 per session.

Closed social network

Working similarly to Facebook, this is a private, completely closed social network specifically for carers. On here you can find peer support, information, events and much more.

Once registered with the Carer’s Federation you will be given access to this site.

Pathways

Do you care for someone who is in the last years of their life and sometimes feel as though you need someone to care about you? Would you like help with the practical things, someone to talk to or just time for yourself? Interested? Yes? Then please read on...

What we offer Pathways Carer Support Service is a fully funded service which is tailored to suit your needs, so the type and level of support you receive, depends very much on what you want.

Below is just a small selection of the kind of things we can do:

• Offer you a short-term respite break (up to 4 hours per week) from caring by providing professionally-trained care support workers.

• Give you someone to talk to about your worries and feelings. This could be in your own home or in the community.

• Help you sort out any areas of concern you may have such as housing, finances, debt, getting aids and adaptations, employment, legal advice and social activities.

• Help you to feel more confident about what you do.

• Help you with identifying and/or accessing other services which may be of benefit to you or the person you care for.

• Help you to prepare emotionally and practically for your bereavement.

• Provision of emergency care and support at a time of crisis.

• Provision of the service, free of charge.

Why we do this

• To enable the person cared for to remain at home with increased carer support, averting hospital admissions due to carer breakdown (as opposed to an unanticipated change in the patient’s condition) and support facilitating discharge home, where that is the person’s preferred place of care.

• To reduce isolation of carers and their ‘cared for’.

To allow carers to have a life of their own alongside their caring role.

• To ensure that carers have access to information and training, and are signposted to other appropriate services.

• To ensure the carers’ spiritual requirements are met including the ability to attend religious services or other spiritual/cultural activities.

• To provide independence and personal dignity for people approaching the end of life

Who can access this service?

• You and the person you care for must be over the age of 18.

• The person you care for must be registered with a GP Practice in Nottinghamshire or Bassetlaw.

• Pathways Carer Support Service will offer support to a carer who is caring for someone with any terminal condition.

Contact us

You can contact us Monday to Friday, 9am - 5pm.

A Pathways worker will contact you within 5 working days of the receipt of referral.

If you would like to find out more about the service, please contact the Nottinghamshire Carers Hub on: 0115 824 8824

We give expert advice, information and support. Caring can be extremely complicated. The maze of rights and entitlements can be complicated. Filling in paperwork can be complicated. Getting a break can be complicated. Our feelings about caring can certainly be complicated. We're here to make sure that no matter how complicated your query or your experience, you don't have to care alone.

Our expert telephone advice and support service is here if you want to talk about caring. If you're looking for answers, our online information and support is the best place to start.

We connect carers so no-one has to care alone. Every day 6,000 people become carers. It can be bewildering if you don't know where to turn for help and support. Through our carers' groups and volunteers we're reaching out in local communities throughout the UK and online.

If you're new to caring, we want to make sure you get the right support from the start.

Connect Service

The Connect service is aimed at people who might be struggling with keeping active or managing personal affairs, or who might be feeling lonely. All these things can happen so easily as a result of getting older, poor health, family moving away or losing loved ones. The Connect service can find information about local services, activities and opportunities. If needed, Connect can work with people around money issues, housing problems, health management and other things. This support will be tailored to each person and will focus on achieving what they want in a way that suits them.
