Description of Service
Ashworth Hospital, Mersey Care NHS Trust jointly provides a national service for patients who require treatment and care in conditions of maximum security. The catchment area includes the North West, West Midlands and Wales.
The admission criteria to High Secure Clinical Business Unit are derived from the National Health Service Act 1977 (Section 4) which requires the Secretary of State to provide special hospitals for detained mentally disordered patients “who in his opinion require treatment under conditions of special security on account of their dangerous, violent or criminal propensities”. Increasingly, new referrals and admissions are direct from the criminal justice route.
The High Secure Clinical Business Unit is a component of a complex range of mental health, personality disorder and cognitive rehabilitation services. Some patients have a lifetime of accessing services with a small proportion using high secure for a part of their pathway of care. Networks and links to key services, agencies and professionals are vital to ensure that the high secure service is connected into the wider NHS, social care and the criminal justice system as a key stakeholder to influence policy and decision making.
Setting Where Work Carried Out
·  Most of the trainee’s work will be carried out at Ashworth Hospital. It has approximately 210 inpatients; all detained under The Mental Health Act 1983. Experience will be gained in assessing and managing patients with severe and enduring mental disorders (mental illness and personality disorder diagnoses).
·  There will be opportunities to undertake assessments of referrals from prisons, other high secure units and medium secure units.
·  The trainee will also have the opportunity to assess referrals from the Courts under Consultant supervision.
·  The trainee will have the opportunity to undertake psychological assessment and therapies.
·  There is a well stocked library with IT facilities on site.
Responsibilities of Trainee
Work in accordance with GMC guidance for doctors.
Engage in the induction process to understand security and administration procedures at the hospital.
Ensure that your training portfolio is maintained, WPBAs are undertaken.
At the start of the post develop a PDP (referring to the core competencies schedule) and agree this with your clinical supervisor.
Attend and participate in ward rounds as timetabled.
Effectively participate and liaise with other members of the MDT in the care and treatment of patients.
Participation in education and audit is mandatory.
Prepare medical CPA reports in good time and attend CPA meetings.
Undertake mental state assessments and reviews following discussion with your clinical supervisor.
Meet with the college tutor for reviews.

Psychotherapy Training
There is a wide range of psychotherapy training available at Ashworth Hospital in a range of modalities including CBT/CAT/MHA and short term focussed interventions. A specific case will, after discussion, be assigned to you.
It is important you maintain the Mersey Deanery psychotherapy logbook.
Details Of Working Out of Hours (on-call):
On-call is resident (there is an on-call room on the Ashworth Site) and covers in-patient units at Southport DGH, The Hesketh Centre and Ashworth Hospital. Further information will be provided at induction. Please inform medical staffing at Mersey Care NHS Trust if you do not have your own transport. It is essential you participate in handover procedures.
a) Late on call (Monday –Friday) 5pm till 10pm (incorporating half hour handover with night time on call).
b) Night on call 9.30 pm till 9.30 am following day.
Compensatory 2 day rest after week of night on-call (Friday –Friday).
c) Weekend and Bank Holiday 9am – 10pm.
One day compensatory rest after long weekend (Friday to Sunday 9am -10 pm).
Frequency 1 in 10 (subject to periodic review)
