YES – Complete this form – this is to be completed when the individual has confirmed they have joined the update service. Return the completed form to HR with a copy of the screen print showing the online check status to
NO – Do not complete this form - Please complete a DBS 1 form instead
· The individuals original DBS Certificate MUST still be seen to complete an online status check,
· The individuals identity MUST be checked on sight of the original certificate or an online status check cannot be completed
· You must be entitled to the same level of DBS Check as on the original certificate (i.e enhanced) with the entitlement to the same barred list checks/workforce
