Shift Differentials:

Work Week: Sunday – Saturday

Overtime Rate(s): Overtime rates do not apply to physician contractors. For other medical professionals 1.4 times the hourly base rate for all hours over 40 in a week, or according to state law in the location staffed, if state law differs from federal law.

Holiday Rate(s): Holiday rates will be billed and paid at 1.4 times the hourly rate.

On-Call Rate(s): On-call rate to carry the pager will be $150.00 per weeknight and $250.00 per day on the weekend.

Call Back Rate(s): If the medical professional reports to facility during on call hours, the regular base rate applies for the hours present at site, with a one hour minimum, unless total hours worked for the week, exceed 40 hours.

Education/License: Undergraduate Program, Medical School (M.D or D.O), Internship, Residency Program, and at least one unrestricted U.S. license

Certification: Board certified or board eligible

Education/License: Undergraduate Program, Dental School Internship, Residency Program (for specialties), and at least one unrestricted U.S. dental license

Education/License: A Baccalaureate Degree in Pharmacy from a college of pharmacy accredited by the American Council of Pharmaceutical Education (ACPE), OR a Pharm D degree program

Certification: Successful completion ofACPE approved continuing education of at least 15 hours annually which maintains pharmacy skills and knowledge in the preceding 2 years.

Experience: Experience as a pharmacist for at least 12 months within

Yrs of Experience in Specialty: Minimum of one year experience within the preceding 3 years unless otherwise stated in the requirements.

Minimum Education: Bachelors Degree (preferred) and graduated from an

License: Unrestricted state license to practice in the U.S.

Certification: Possess certification as a physician assistant as determined by the National Commission on the Certification of Physician Assistants.
