Youth Work Application

1.The Youth Work Application is completed for youthhoused in Youth Development Centers (YDCs) to obtain employment inside of the YDCand in the community.

2.Youth fill in the top section of the applicationand answer the four questions asked to the best of their ability. Theyouth submits the application to his Residential Case Manager (RCM).

3.The RCM schedules a Child and Family Team Meeting (CFTM)for the team to determine if the youth will participate in the work program. The team decides, based on eligibility criteria and youths’ IPP/treatment plan, whether youth are allowed to participate in the work program.

  1. If it is determined the youth does not meet eligibility requirements for the Work Program, the RCM checks the denied box and each of the Treatment Team Members sign the form. The RCM writes the reason for the denial in the comments section at the bottom of the form and returns the form to the youth.
  2. If it is determined the youth meets eligibility requirements for the Work Program, the team adds employment goals and objectives to the youth’s Individual Program Plan (IPP) during the CFTM. The RCM checks the approved box and each Treatment Team Member signs and dates the form.

4.The RCM forwards signed applications for On Campus to the Work Program Coordinator (WPC) for job placement. The WPC reviews, signs and dates the application.

  1. If approved, the WPC checks the appropriate box and proceeds with the job placement process.
  2. If denied, the WPC checks the appropriate box and writes the reason for the denial in the comments section at the bottom of the form. The WPC returns the form to the RCM.

5.The RCM holds signed applications for Off Campus work until youth are near the end of the Career Readiness (CR) Program. Within two weeks of the youth completing the CR program, the RCM forwards the application to the Superintendent and senior manager team for approval.

  1. If approved, the Superintendent or senior manager checks the appropriate box and proceeds with the community job placement process.
  2. If denied, the Superintendent or senior manager checks the appropriate box and writes the reason for the denial in the comments section at the bottom of the form. The administrator returns the form to the RCM.
