Information About Your Web Environment

1.  Briefly describe your web environment and what you would like to protect using CUAccess:

2.  List the protocol, hostname, and port of the machine you would like to protect with CUAccess? List all that apply (example: http, tristan.colorado.edu, 80).

4.  Do you agree to the ITS minimum-security standards (www.colorado.edu/its/security/csrweb.html)? All web servers will undergo an ITS security audit prior to installation.

5.  Web server architecture (clustered machines, virtual hosting, are protected pages separated from public pages in web server’s file system, etc):

6.  Does your web server have domains that will not need to be protected by CUAccess? (i.e. will you have any pass-through domains?)

7.  Please describe the people who will need access to protected pages on your site and how they are affiliated with CU-Boulder. (Are they visiting faculty, active faculty, current staff, student employees, graduate students, alumni, etc.?) Do different individuals, groups, or roles need varying levels of access to your web site? If so, please describe.
