The RESOLUTIONARY Awards celebrate everyday heroes who are absolutely committed to preventing and ensuring the safety of all women against violence. The RESOLUTIONARY Awards honor individuals who stand tall everyday in a way that cultivates change as well as inspires and motivates others to join in the efforts to eradicate all forms of violence against women; thus forging paths toward a brighter future that is safe and just for all its members.
The Susan H. Horwitz Memorial RESOLUTIONARY Award will recognize the individual contributions of ONE woman and ONE man working in fields related to intimate partner, family and sexual violence who demonstrate exemplary vision, leadership and determination.
The Bruce Frazee Memorial RESOLUTIONARY Award will recognize the efforts of ONE individual who is a grassroots champion working to end violence against women in our community.
Nominations from previous years are welcome and may be refreshed and resubmitted.
NOMINATIONS WILL BE ACCEPTED: December 6, 2017 – January 12, 2018
RECIPIENTS WILL BE ANNOUNCED: February 5, 2018

RESOLUTIONARY AWARDS ELIGIBILITY CRITERIA:
1) Individual is committed to eradicating and preventing violence against women and has demonstrated this commitment through his/her actions; please provide specific examples.
2) Individual demonstrates leadership, dedication and determination; please provide specific examples.
3) Serves the Greater Rochester Area
(Monroe, Livingston, Ontario, Wayne, Orleans counties).
4) Current or previous Board Members or Staff of Safe Journey/Stand Up Guys/Resolve are ineligible. Members of other Boards and agencies are encouraged to participate.

Narrative:
In no more than 750 words provide specific, concrete examples of how the nominee demonstrates leadership, dedication and determination in their individual efforts toward eradicating and preventing violence against women. Provide a detailed narrative explaining how the nominee's service is outstanding, significant, "above the call of duty;” and how their heroic efforts impact the eradication of violence against women.
References: Please provide names, titles and phone numbers of two (2) other persons who are familiar with the nominee's contributions and are willing to be contacted, as necessary, by the Award committee. (In lieu of Letters of Recommendation)
Letters of Recommendation: Letters are not necessary, but up to three (3) letters of recommendation will be considered by the committee.
