Warm up:

Facilitate discussion.

Explain that in today’s lesson, the class will go through each section of a job

application, so that they will know how to fill it out correctly.

What?

What information is required in each section?

Pass out Worksheet # 1: Job Application Form,

TERMS TO KNOW: Going through each section, identify and define the following vocabulary words and describe what information is required:position applied for, availability, shift preference, supervisor, and most recently attended (school), currently enrolled, reason for leaving, post secondary.

Step by Step

Today we are going to learn how to read and fill out a job application. This

will help you when the time comes for you to fill out a real application for a job. Also, today’s lesson will help you learn how to answer several questions found on the job application.

ALL APPLICATIONS MUST BE FILLED OUT IN BLACK INK

Pass out Worksheet 1: Job Application Form.

First, take out your job aid (ThisIdentifies personal information, and uses correct spelling)

Say: What is this a picture of?

Response: A job application form.

Look at the job application below. What information is needed in thefirst section?

This is NOT about any of the store’s information. It is about “who” is applying for this position. (Allow the learners to come up with the answers) Fill in your first section on your worksheet. Use your “job aid” to fill out the required information correctly with no errors.Complete with your name and address.The business requires yourentire real social security number as this is not a practice form when applying for a position.

Look at this section on the application. What does availability mean?

(Allow the learners to come up with the answer).

Availability means the days and times you are able to work. If you get out of school at 2:15, there is no way you can be on the job site at 2:15. You MUST put 3:00 or3:30 as the earliest possible time for arrival. Date available: When can you start? Please write “immediately”.

What does shift preference mean? First tell me what does the word “prefer” mean?

Response: Prefer means to like one thing better than another thing. Shiftpreference is asking whether you like to work during the day, evening orovernight. We call these options“shifts”, the evening shift and the night shift.

Here you write which time of day you would most like to work. Forexample, I prefer to work the afternoon/evening shift because I am in school during the day. On weekends, we are available to work “ANY” time. Please put the word ANY on either or both Saturday and Sunday. Some may have commitments on Sundays with family and you must account that you would not be available until after 11 am or after noon.

Are you at least 16 years of age? They want to know if you are older than 16 years old

Next, Salary Expected.

If you put a figure in this block, the employer will assume you will not work for any less and your employment application may be overlooked. We all would like to work for $20 an hour, but it is not practical. What is the current minimum wage?

Response: $7.68an hour and $7.00for tip paid employee

It is best practice to place “Negotiable or Open” in this area. This means you are willing to discuss it at the interview.

Next, Position Desired or occupation. You do not put the store’s or company name in this box. It is asking what job you wish to apply for. (cashier, stock, sales, ticket taker, associate service, maintenance, or clerk)

Are you legally authorized to work in the United States? By saying yes, you declare you are not an illegal alien. (Not a US Citizen)Were you born here? Can you provide documentation you can work in this country?

You will also need to have a current E-mail address: You should have it on your job aid. Employers may contact you for a job interview or to request further information.

Section two. EDUCATION AND TRAINING.

What do you think “most recently attended”or“nowenrolled” means?

Response: Attend means to go somewhere, in this case, school. Most recently

attended asks: which school is the last school you went to?

For all of you, it is this school!) Are you enrollednow? This means the same as now attending. (Two words that have thesame meaning are called . . . synonyms.)

Remember that this section is asking about your schooling. Again, your job aid will correctly spell the name of your school, the address and the telephone number if you need it.

What about last grade completed? This is asking about the last grade you

FINISHED in high school.You may list the numbers such as 9th, 10th,11th, or 12th.

9-Freshman , 10-Sophmore, 11-Junior, 12-Senior.

You can all identify/write that you are takingclasses in the Occupational Course of Study Program. Employers would not understand the letters OCS. It also asks, did you graduate? You need to answer yes or no. In small letters, you may write the year you intend to graduate to show the employer you are currently attending High School.

“Location” on some applications refers to the City, State where the high school is located.

Post Secondary- refers to information about attending college. You have NOT had any college courses.

I would not check off“graduated”. That doesn’tapply. During an interview, you can inform the employer you’re intending to finish high school.

Section Three. EMPLOYMENT

“Most Recent Job, Employment history”.

What is the title of this section?

Response: Employment History.

  • Do not put N/A in every box. Please print: “No work history”

What does reason for leaving mean? On here, does leaving mean youwent home after work?

Response: No! It means you left the job. You stopped working for that company. Here you tell why you stopped working there.

What are some possible reasons for leaving?

Response: I moved. I quit. I got laid off. I was fired. I took a new job.

SALARY

What is salary?

Response: $ or pay per hour, how much money you make.

Some of the application asks…. What are the dates worked? What does that mean?

Response: Start date and end date: from – to—

Also, from --- to “present”(until now, still working there)

Why is salary mentioned twice? Starting salary and final salary?

Response: The employer wants to know if you received a pay raise

SKILLS

Please list any skills that you have that are related to the position you are applying for. You can add skills such associal skills, working with people andhelping them, manual abilities working with your hands, computer skills, prior experience through volunteering opportunities such as stocking, raking, and customer service, work well in teams, good communication skills, self-advocacy, leadership, artistic, work independently and can make change, are a few.
