2003-2008School-Based Adolescent Care Coordinator

●Provided case management services to adolescents and their families at the Wayne Township Alternative School and Middle School

●These duties included, but are not limited to: providing holiday assistance for low income families, co-facilitated anger management groups and a women’s group, employment training, pregnancy and parenting education, provided advocacy at school and at home for students and provided support, self-esteem empowerment for them to achieve even their smallest goals.

●Involved in planning for school based services and strategic planning for two different committees for Cummins as an organization, and for our school based team.

●Co-developed a pamphlet used for our teenagers informing them of our services and a “What to Expect:” guide.

●Current co-facilitator for our Care Coordinator meetings which involves school based and Marion County care coordinators with over 15 members. These meetings include staffing of cases, collaboration, trainings, and collecting community resources.

●Co-facilitated and developed an after school Intensive Outpatient Program for children ages 8-13 during my first year at Cummins. This group focused on anger management skills, a parent group component, feelings, coping skills and communication.

●Also co-facilitated an expanded version of IOP during the summer months. This program targeted children from 8-17 years old. Planned outings and outlined treatment planning for the program.
