SUBCONTRACTOR ORIENTATION CHECK LIST

Subcontractor’s representative (Competent Person) will check off items and sign his name on the bottom of the form. Superintendent shall retain a copy.

_____Give a copy ofSite Specific Safety Plan subcontractor’s representative (Foreman).

_____Weekly Safety Tool Box talks will be held with all workers on site and documentation given to Superintendent.

_____Submit writtensite specific safety and hazardous communication program with SDS’ to superintendent prior to starting work.

_____Workers shall wear hard hats with bill forward, safety glasses, reflective vests or high visibility clothing in the designated work zone. Workers will have on their person hearing protection (ear plugs).

_____Jobsite cleanup will be done daily and trash put into dumpsters.

_____Foreman (competent person) shall conduct documenteddaily safety inspections of their work areas.

_____Scaffolds (if used) shall be erected, inspected (tagged) and maintained according to OSHA Construction Standards 29 CFR 1926.450 - 454

_____Masonry workers shall wet cut concrete or masonry materials when feasible. Use of mechanical ventilation to remove respirable dust from the workers breathing zone is required when dry cutting masonry materials.

_____All operators offorklifts shall be licensed. Operators of mechanized equipment, scissor lift platforms and articulating boom lifts shall be trained and certified and follow manufacture’s safety requirements.

_____All employees who are exposed to falls greater than 6 feet above a lower level including scaffolds and steel erection shall be protected. Specific fall protection requirements shall be discussed with Superintendent and Safety Director prior to beginning work.

_____Foreman shall submit to Superintendent, certification of employee training for fall protection, scaffolds, ladders and project safety rules. Contractors engaged in excavating shall submit certification of employee training.

_____Subcontractors performing hot work in an enclosed building shall obtain a hot work permit from Superintendent.

_____Subcontractors shall have in their possession a first aid kit, fire extinguisher and provide fresh drinking water daily. Competent Person shall inspect first aid kits for missing items and replace as required.

_____Subcontractors shall ensure that all materials and equipment are secured before leaving the jobsite each day.

_____Electrical subcontractors shall inspect GFCI’s daily and provide documentation to Superintendent.

_____All flexible cord sets on the jobsite shall be #14-3 or greater and rated extra hard use. Replacement plugs shall be suited for outdoor use.

_____Superintendent shall discuss with Foreman the jobsite Emergency Plan in case of an emergency situation.
