NOTICE OF DISCLAIMER

We have read and understood the terms, conditions and rules of the FMM Penang Rock N Roll Futsal Tournament 2013, and hereby agree to abide and adhere to the rules and regulations that has been set by the organizer. The organizing committee or Federation of Malaysian Manufacturers will not be held liable and responsible for any injuries sustained, loss of personal belongings and any form of physical damages to the players and participating team throughout the duration of the tournament.

Attached is the cheque / postal order / Bank Draft/ Bank in Slip for the amount of RM ...... payable to “Federation of Malaysian Manufacturers” as the registration and entry fee of the team.

Entry fees can be banked into FMM HONG LEONG BANK BERHAD account No: 04300009855 and the payment slip is to be attached to this registration form as proof of payment.

·  All completed registration form together with entrance fee must reach FMM Penang by 18 September 2013.

·  Registration is limited to 120 teams on a first-come-first-served basis

·  Acceptance of entry will be at the total discretion of the organizer. Each team is to be represented by a team manager who shall head the team delegation as the team leader

TOURNAMENT

RULES AND REGULATIONS

1. The Rules and Regulations of the Tournament will be in accordance to ”Permainan Futsal Persatuan Bolasepak Malaysia” and any other conditions laid down by the tournament officals.

2. Each team must consist of 8 participants; 5 players dan 3 reserves. Only players listed in the official registration form are entitled to participate in the tournament. No player can be replaced during the tournament unless otherwise agreed by organizer. Each player is only allowed to register in one team in one category only. If a player loses or is disqualified in the league stage, he/she is not allowed to replace another player in the knockout stage.

3. The competition will follow the fixed schedule. Teams will have to report themselves to the filed officers 10 minutes before the scheduled match. There will be no extra waiting time. If any teams fail to show up on the event day, the organizer has the right to replace a team that withdraws with any team deemed fit to continue with the tournament and the entry fee is not refundable.

4. The duration of the match will be 10 minutes per game with 5 minutes each half without break.Substitution should not exceed 1 minute. A player that has been substituted can be recalled into the court.

5. Competition Pattern: Early rounds will consists of 3 leagues in one group. Only the champion will proceed to the second round, that will be played knockout style. If there is a draw at full time, a ”sudden death” penalty shootout will determine the winner.

6. Teams are responsible for the behavior of their players, officials, members, supporters and any person carrying out a function at a match on their behalf. The organizer will not tolerate any act of violence. Such indiscipline will automatically result in the errant player or/and team to be expelled from

the tournament.

7. All decisions made by the judges/referees are final.

8. The organizer reserves the right to use the name and / or pictures and / or video players before, during or after the competition for the purpose of publicity and advertising, and participants are not

eligible to claim ownership or any other form of compensation for the material.

9. Information on the schedule and time of registration for the matches for all categories and detailed games regulations will be emailed 4 days after the closing date of entry to the registered teams.

10. It is compulsory for every player to wear a numbered jersey, socks (stockings) and ‘shinpad’. Studded football shoes are not allowed.

11. The organizer has the full right to change the venue, date, schedule, format, rules & regulations, prizes, registration fees and contents of the registration form without prior notice given to participating teams. However, registered team will be informed on the changes.

12. The organizer or any parties involved in the tournament shall not be responsible for any expenses incurred by teams, participants or individuals and for any injuries or loss occurred during or after the tournament.
