PATROL REGISTRATION FORM

Dear Guiders,

We are so pleased you and your patrol will be joining us for this year’s Fall Fun Day. It will be a great day of camping activities, nature and friendship at Wa-Thik-Ane in Morin Heights.

To register, complete the Patrol Registration Formsand email the completed copy to: fax it to the provincial office at 514-933-7591.

Please read this letter carefully before completing forms, to ensure they are completed properly.

  • Before you start read the whole form through.
  • The grey fields indicate a fillable area. The form will calculate the amount your patrol must pay.
  • Please ensure you use the proper patrol registration for your branch. The orange form is Sparks/Brownies and the blue form is Guides/Pathfinder/Rangers.
  • It is important that all names and iMIS numbers are printed clearly or typed.
  • Be aware that the bus cost is only waived for a Guider with a full patrol. (As per ratio for the specific branch) Guiders having to pay can ask their district or unit to defray the cost.
  • We have a 400 participant cap for this event, submit your forms as soon as you can.
  • The registrations will be date stamped and entered by date submitted.
  • If payment is incorrect you will be notified and your registration will be on hold.
  • There will be NO payments collected at the event- all forms and payment must be in by the deadline of Monday, September 26th at 9 a.m.
  • Make sure Guiders and volunteers have a PRC.
  • Please remember you will need one adult volunteer on the bus who is first aid certified in IMIS.

Please note that no refund will be given after the registration deadline of Monday, September 26th without a doctor’s medical note.

Please note, the event is first come, first served – we have a 400 person cap on registration. Watch your inbox for more details. An eblast will be sent to all Guiders by the end of August and will include the SG 1 and SG 2 forms.
