·  All Petitions must be completed in Microsoft Word and submitted as an attachment to .

·  The advisor recommendation is not to be completed or requested by the student.

Your petition will only be reviewed if you have:

Fully complete the form below- the petition must be typed and submitted via Microsoft Word documents (no PDF, faxes or hand submissions).

On the Explanation sheet of the application, please be sure to:

a.  Give a detailed explanation of all pertinent information including why you are requesting that action be taken.

b.  Be sure to include a plan of action:

i.  If you are requesting a third time repeat of a course, explain how you intend on performing up to University standards if given the opportunity to take a class for a third time.

Attach any necessary supporting documents. (i.e. general studies check-sheets, course syllabi, etc.)

Course Overload Requests:

·  You must submit a new petition each semester that you wish to take an overload.

·  You must submit the Course Overload form found https://cisa.asu.edu/standards_petitions, in addition this petition.

·  Approval of an overload petition does not imply that you may enroll in a class after the add/drop deadline or that you may register for a class section that is already full.

·  These criteria provide guidelines for approval. Requests from students who meet these criteria may be approved or denied depending on additional factors. Requests from students who do not meet these criteria are much less likely to be approved. Taking an overload is a privilege granted when past performance demonstrates your ability to do well.

Overload requests will be considered

Third Time Repeats

·  You may not take a course three or more times if you have passed that course with a “C” or higher.

·  Use one petition per course requested.

·  All prerequisites for the course must be passed with the required grades before submitting this petition (even if you have been allowed to register previously without them).

·  Approval to retake a course does not imply that you may enroll in a class after the add/drop deadline or for a class section that is already full.

Request General Studies designation or requirements

·  Use one petition per course/requirement area requested.

·  Requests for general studies credit may be approved for transfer courses or for ASU omnibus classes (eg. 294, 394, or 494 classes) which are not reviewed by ASU’s General Studies committee and thus do not carry pre-approved general studies designations. Requests for established ASU courses are discouraged; typically established ASU courses that are not already designated as general studies courses either do not meet the criteria or the department has decided not to submit it for general studies designation. Sometimes an instructor will insist that his or her class meets general studies criteria, but if the university general studies committee has decided otherwise, the petition will not be approved.
