Residential service / l  The Administration, service agencies and the family members of PWD should work hand in hand to care for PWD and to encourage and assist PWD living in the community
l  Residential service and community support service for PWD should be developed in parallel to meet the needs of PWD
l  Develop diversified residential service in order to cater for the different needs of PWD
l  Promote collaboration of the public and private sectors, and encourage multi-sector participation and commitment
l  In line with the Hospital Authority’s cluster system, encourage service agencies for ex-mentally ill to form district-based clusters with hospital in the same district / l  Increase the number of residential places
l  Conduct assessment for PWD on the waiting list for residential service with a view to shortening the waiting time for people with genuine need
l  Establish a continuous assessment mechanism and a timely discharge system for those staying in hostels; while in parallel set up a fall back system such that parents know that their disabled children could be provided with residential care when in need
l  Strengthen day care and community support service so as to reduce the need for residential service by the PWD
l  Consider better utilization of cross sectoral /departmental resources, such as use of empty hostels of special schools during vacations, vacant school premises, public housing estates to provide residential service for PWD
l  Encourage the development of self-financing hostels and provide suitable support
l  Tackle aging population problem, improve the hostel facilities and set up integrated service centre, to implement the “aging-in-place” policy to cater for the needs of PWD at different stages
l  Explore the provision of visiting medical service and alleviate the shortage of nursing staff
Day care and community support services / l  Continue to develop day care and community support services: strengthening personalized service and support to carers, community support network and multi-disciplinary support to assist PWD to integrate into the community
l  Promote tripartite partnership of the public, commercial and the community, to explore and utilize social resources creatively to provide support and service for the PWD / l  Strengthen integrated family support service for the severely disabled people living in the community
l  Provide people with severe disabilities with transitional residence, day training, nursing and support services to facilitate their return to the community after discharge from hospital
l  Strengthen the support and training for the carers of PWD
l  Enhance the functions of District Coordination Committees
l  Assist PWD living in the community by granting cash allowance, if resources permit so
Development of self-help organizations / l  Continue to support the development of self-help organizations and assist them by providing a collaboration platform with the commercial sector and promote cross sectoral partnership
