UNIVERSITY OF HAWAII BAND EVENT PARTICIPATION AGREEMENT AND RELEASE

In consideration for me (if participant is 18 years of age or older) or my child's (if participant is under the age of 18) participation in the 2017 UH Bands High School Jr/Sr Night at the UH Football game on September 30, 2017, I agree to the following:

Publicity and Talent License and Release: I do hereby give the University of Hawaii, its officers, directors, agents, employees, affiliates, and successors the unrestricted right and permission, in perpetuity and throughout the world, to use participant's name, voice, likeness, biographical data, and musical performance, in any and all media, whether now known or hereafter devised, in connection with participant's participation in the event, for promotional and fundraising purposes only. Such uses may include, without limitation, use on or in websites, emails, other marketing materials, grant proposals, and promotional and fundraising CDs and DVDs.

Medical Consent and Release: For participants under the age of 18, in the unlikely event that my child becomes ill or is injured during the course of the event and I or my child's physician cannot be immediately contacted, I authorize and direct the University of Hawaii Band staff to send my child (properly accompanied) to the hospital or physician most easily accessible, consent to any necessary and appropriate emergency treatment, and release the University of Hawaii, its officers, directors, agents, representatives and employees from any and all claim of liability in connection therewith.

Liability Release: I hereby release and hold the University of Hawaii and the participant’s respective high school, their officers, directors, agents, representatives and employees harmless from any and all claims, liability, causes or assessment, of whatever type or nature, for losses, damages or injuries to participant arising out of, during, or in connection with the participant's participation in the event.

Statement Regarding Copyright in Sheet Music: I understand that it is a violation of copyright law to download, reproduce or duplicate sheet music for musical compositions that are under copyright without a license to do so.

I have carefully read each of the statements above and understand and agree with each.
