There is not a quorum. Committee will operate as a sub-committee with two members attending.

No representative from the public was present, no comments were made.

Review/correction/approval of previous meetings’ minutes

The January 24, 2015 minutes were reviewed. They were not approved due to lack of a quorum.

Remarks from the Office of Human Rights- Regional Advocate Report

-Mr. Daye went over the new procedures forLHRC affiliation of a new service within the same region. He went over the required “additional “documentation to be sent to the LHRC members and his office. Check the DBHDS website and follow the instructions outlined in the February 02, 2015 memorandum posted from the Licensing and Human Rights Directors.

-Mr. Daye asked that the providersnot to e-mail reports to him or the members of the LHRC.He expressed HIPPA concerns since the reports may contain client specific information.

-Providers were again reminded to mail all reports and agenda items for review to OHR and committee members, no less than two weeks prior to the scheduled quarterly meeting. Failure to do so may result in the items not being accepted for review at the meeting. The program will have to present the report at the next scheduled meeting. Also, if a provider has no representative at meeting, this will be reported to the assigned licensing specialist by Mr. Daye, with a request for citation.

-Mr. also is requesting that the provider reports not be faxed to his office but mailed. He does not have clerical support and there is no one to retrieve the reports from the faxmachine.

-Signed Cooperative Agreements must be up to date. Must include staff added/staff removed. Representative must be an employee of your company. Any changes in agreement; bring two copies of agreement to meeting to be signed by the Chairman.

-Restraints and definitions of what constitutes a physical and chemical restraint were reviewed with the providers. Providers was advised to review human rights regulations and ensure their policies/procedures include only what applies to their program.

-Fire safety:Be sure that yourstaff knows how to use the fire extinguishers. Make sure the smoke detectors are operable at all times.

-There will be a regional training for local human rights committee members on May 13, 2015 at Southeastern Virginia Training Center. Providers can attend, if space permits. Contact Ms Haqq at (757) 253-4066 to sign up.

Old Business

Ms Roberta Hansel was appointed to the PRLHRC to serve from 04/17/2015 until 06/30/2018.

New Business

A make up session was scheduled for May 15, 2015 same time and location. Secretary liaison to notify absent committee members Whitfield, Davis, Hansel, and providers; CJAB, Anne’s Group Home, The Learning Ctr., Hodges Ferry Manor, House of Virtue of make-up meeting to present policy/procedure changes additions and/or reports due.

Ms. Bell of Child of God announced that she will be relinquishing her post as secretary/liaison at the close of 2015. Ms. Sabrina High of Virginia Beach Methadone Clinic volunteered to assume the responsibilities of Secretary/liaison as of January 1, 2016.

Executive(Closed) Session

11:15 am Committee voted pursuant to Va. Code 2.2-3711A to go into closed session for the purpose of discussing complaints, allegations of abuse/neglect and/or behavior plan issues with Virginia Beach Methadone Clinic, and AA&G Group Homes.

11:50 am committee voted to come out of closed session pursuant to VA code 2.2-3711A. Each member then so certified that only the cases of complaints, allegations of abuse and/or behavior planswere discussed with Virginia Beach Methadone Clinic and AA&G.

Recommendations: no recommendations were made to these programs.

The LHRC motioned and approved the following recommendations for Blue Ridge;
