Do they have a Statement of Special Educational Needs? How are they doing at school? What support might they need in future in order to succeed?

You must say if the child has any birth siblings, what their names are, DOBs, give descriptions of their physical appearances and personalities. A brief summary of who has looked after them, where they have gone to school, who will be looking after them in future (or what the plans are). Are the child’s siblings ‘looked after’ or provided with accommodation under section 59(1) of the 1989 Children Act? Are there any Court orders with respect of the child’s siblings and if so, which Court made the order, what order has been made and when? Are any of the child’s siblings also being considered for adoption / permanent fostering?
Wishes and feelings of the child’s parents or guardian
You must include the child’s parent or guardian’s wishes and feelings regarding (the matters set out in regulation 14(1)c), including the father’s wishes and feelings if regulation 14(4)a applies.
If you cannot include the wishes and feelings of the child’s parents (including the child’s father) or guardian, you must say why not.
