2017 ASPIRA Hostos Coqui and SOL Camp Registration Packet

Important Registration Information:

• Applications are processed on a first come, first served basis.

• A non-refundable deposit of 2 Weeks of Camp Cost is due at the time of registration.

• Registration packets must be complete or will not be accepted.

• All Final Payments are Due By June 24th.

2016 Summer Camp Fees

Coqui Camp (K-5th)

  • Regular (6 weeks) – M-F (8-4:30 pm) - $375 ($25 discount with each additional sibling)
  • ASPIRA Employee - $270 ($20 Discount for Additional Siblings)
  • Weekly
  • Coqui Camp – Regular -- $62.50 weekly fee per child/$375
  • Coqui Camp – ASPIRA Employee - $45 weekly per child/$270

SOL Camp (6th - 7th)

  • Regular/Employee/Weekly – M-F (8-4:30pm) - $270 per child (Discount only applies to Sibling Attending Coqui Camp)

ASPIRA Career Explorations (8th)

  • Regular/ASPIRA (6 weeks) – M-F (8-3:30pm) –No Fee for Program

For interested parents, a NON-refundable deposit of 2 weeks of the camp cost only will be required at the time of enrollment per child. (All Payments are due by friday, June 24th)

Summer Camp Parent Checklist

• Your camp fee includes the camp day from 8:00 AM – 4:30 PM

• Field trips • Recreational swimming

• Arts & Crafts, Swimming, Sports, and Games

CAMP POLICIES:

• A current Program application is required for every child in Camp.

• A 2 week non-refundable deposit (See Chart) is required for all registrations. (See Fee Chart).

• Deposits are due at the time of registration and Full payment is Due Prior to Start of Camp.

• Any child picked up after 4:35PM will receive a $1.00 late fee per minute, fees must be paid before they return to the camp program.

• A refund will be issued, excluding the deposit, if written cancelation is received prior to June 10, 2016.

• All medical forms MUST be on file before your child can begin summer camp.

• I understand my child will only be released to those individuals listed on the emergency contact/parental consent form. Valid photo identification – driver’s license must be presented at pick up.

• I understand that a custody order must be on file at the program if one or more parent does not have visitation rights. A custody order cannot be enforced if ASPIRA Children, Youth, and Family Services does not have a copy of the order on file.

• All electronics, games, cell phones and any other item of value will not be brought to camp. I understand the ASPIRA Summer Campstaff will collect and store equipment, but will not be held responsible for lost, broken, or stolen items.

By signing below, I acknowledge that I have received the Parent Handbook and consent to ASPIRA Camps enforcing all of the policies and procedures stated therein. No child will be admitted into camp until all paperwork is completed and returned to the ASPIRA. I have retained a copy for my records.
