Please cover the following points:

□Change over date is 1st January from then on need to make sure use the new paperwork and get rid of all the old documentation.

□Pharmacist needs to read service specification and make sure all staff know how to operate the scheme.

□It will no longer be appropriate for non-trained staff to provide this service.

□Pharmacies newly signing up to the service must complete all relevant training prior to starting service provision.

□There is a change over period of 24 months for pharmacies already providing the service, during which time staff who provide this service must be registered and complete the relevant training.

□All pharmacists providing the scheme must complete the relevant training by the end of June 2011 and provide copies of certificates to Anna Swift.

□All technicians providing this service must be registered by December 2011 and have completed the relevant training by January 2012. Copies of certificates of completion must be sent to Anna Swift.

□Any locums providing the scheme and who have not completed CPPE training pack must complete the locum agreement form in each pharmacy where they provide this service. Those working regularly within pharmacies in NHS ALW must complete the CPPE training and send a copy of their certificate to Anna Swift bythe end of June 2011.

□There must be a pharmacist on the premises when operating this scheme.

□It is the pharmacists responsibility to ensure their staff are suitably trained to provide the service, know when to contact the SMT regarding a patient, are able to provide appropriate advice and signpost to relevant services.

□They will need to review their SOP to make sure it still fits, this SOP needs to be reviewed every 2 years.

□When new clients are initiated on the scheme the Substance Misuse Team will ask the client to sign the Client Contract Form and provide the pharmacy with a copy of this form.

□Staff must confirm the identity of patients when they start on the scheme and subsequently if necessary, preferably by checking some form of ID. If this is not possible the substance misuse service should have provided a description of the client which can be used for ID.

□The substance misuse team will contact the pharmacy prior to sending the patient for the service. If the patient fails to present then pharmacy staff must inform the substance misuse service. If patients present to the pharmacy who the substance misuse team have not informed the pharmacy of the patients practitioner should be contacted prior to dispensing the prescription. They will then either authorise the pharmacy to dispense the prescription or advise the patient to go to the agreed pharmacy or present to the substance misuse service. It is important the patient attends the agreed pharmacy as this is part of the clients agreement with the substance misuse service and shows they are willing to work with their practitioners.

□Pharmacy staff must contact the substance misuse service prior to dispensing prescriptions if:

  • The client has failed to collect for 3 doses
  • The client has repeatedly missed doses eg misses several times during the week
  • The client appears intoxicated
  • The client appears ill
  • There are problems with the prescription

□Pharmacy staff must contact the substance misuse team if:

  • The client does not consume the whole dose under supervision
  • The client tries to avoid supervision
  • The behaviour of the client is unacceptable and contrary to the Client Contract

□Claims paperwork has been significantly changes so that multiple patients can be entered onto the same form, they must mark the form indicating whether methadone of buprenorphine has been supplied and if the patient was supervised, given a take home dose or did not attend.

□They need to ensure all paperwork is filled in properly; the names of all staff supervising consumption must be detailed on the monthly form.

□All claims forms should be submitted to

Those pharmacies in Leigh should submit to this address not the Leigh substance misuse team.

□All paperwork should be submitted promptly as this ensure practioners are aware of patients who are not collecting all doses from the pharmacy.

□Staff must complete all controlled drugs registers as well as the paperwork for this scheme.

□Staff must report any incidents to SMT ASAP. A report form can be completed if necessary and should be returned with monthly claims form.

□Pharmacists do not have to go on PCT training to provide the scheme as long as they follow SOP, SLA and service specification and have completed the CPPE training. Locums who work infrequently at the pharmacy do not have to complete the CPPE training although this is strongly recommended.
