Job Title: RN, Clinical Care
Exempt Status: / Non-ExemptSalary Information: / Commensurate with experience
Supervision: / Reports to Director of Clinical Operations but follows direction of Lead RN.
Work Location: / Point Reyes, Bolinas
Purpose of the Job: / The Clinical Care RN is responsible for direct and indirect patient care and education.

- Provider visit-support
- Patient histories: Perform histories (“tee up”) on all new patients and patients with sentinel visits not seen in > 1 year.
- This should be done in advance as much as possible (keep a schedule with patients who have been teed up for at least a week ahead; RN should communicate any necessary info during huddles or by phone to other sites
- Provider-minimal visits: Tee up patient visits during high capacity shifts
- Triage walk-in emergencies; Flip visits.
- Patient education as needed (i.e. diabetes, diet, exercise, cholesterol, etc)
- PPD readings per protocol
- Patient education and follow-up
- Provider ancillary work-support
- Refill medications per protocol
- Monitor “E” (e-refill)jellybeans for all providers: Refill meds per protocol
- Refill all faxed/called requests for meds per protocol
- Review and triage lab results per protocol
- Order Labs per protocol
- Provides phone triage and advice
- Transitional Care
- Identify our patients who have been hospitalized and/or seen at ER and ensure rapid follow-up with office visit, home visit, or phone contact (i.e. review discharge summaries)
- Complex care Management
- Work with our providers to ensure follow-up and follow-through with treatment plans for our most complex and vulnerable patients
Qualifications and Requirements: /
- Good knowledge of the nursing process and critical thinking abilities
- Knowledge of electronic medical records and tracking systems
Education and/or Experience: /
- Two years experience with direct patient care and coordination preferred
Language Skills: /
- Preferred: Bilingual Spanish and English
Other Skills and Abilities: /
- Ability to communicate clearly and effectively through verbal, written, and electronic means
- Ability to work independently as well as a member of the medical team
- Ability to follow policies and procedures
- Ability to operate computer, printer, PDA’s, other office machines
- Ability to communicate clearly verbally and in writing
- Demonstrates good listening skills
- Good organizational skills
Licenses, Certifications, Registrations: /
- Current California RN license
- Current CPR-BLS certification

- reach with hands and arms, grasp objects, push/pull file drawers of 10-50 pounds, twist the upper body, rotate the wrist, and use finger dexterity writing and using the computer.

- lift 10 to 20 pounds from floor to desk or top of file cabinet, and carry for a distance of 10 to 40 feet;
- bend to file or retrieve an item on the floor or in a drawer;
- stand for several minutes at a time for up to 1 hour a day;
- walk for a distance of 10 to 40 feet up to 1 hour a day;
- climb stairs.
