I/We herebyagree to indemnify and hold harmless The Lutheran Church of St. Mark, their agents and employees and volunteers against any and all liability, claims, suits, losses, costs and legal fees caused by, arising out of, or resulting from any act or omission of the applicant including the negligent acts or omissions of its employees, agents and/or assigns.
I/We further agree to be responsible for the payment of any fees and labor charges, and any other additional costs or fees incurred by us in complying with the use of facilities.
ALL APPLICANTS REQUeSTING MORE THAN A “ONE TIME” USAGE MUST PROVIDE A CERTIFICATE OF INSURANCE THAT INCLUDES;
GENERALLIABILITY INSURANCE WITH A MINIMUM LIABILITY LIMIT OF $1,000,000; FROM AN INSURANCE COMPANY WITH AN A.M. BEST RATING OF A- OR ABOVE; THE LUTHERAN CHURCH OF ST. MARKis to beINCLUDED AS AN “ADDITIONAL INSURED”.

THE LUTHERAN CHURCH OF ST. MARK
The Lutheran Church of St. Mark is proud to be part of the Glastonbury community and, when possible, allows use of its facilities to members and local groups for approved events.
The completed form will be reviewed by the church leadership and the applicant will be notified of the decision. Applications should be submitted three weeks prior to the date of use.
Congregational activities of The Lutheran Church of St. Mark have priority for the use of facilities.
Returning, regularly scheduled groups are given priority over new applications for the same dates and times.
While St. Mark will not charge a fee for profit for the use of the facilities, applicants may make a contribution to help defray Church expenses for room preparation, maintenance, utilities and janitorial services.
GENERAL RULES FOR USE OF CHURCH FACILITIES
1.An Application for the Community Use of Facilities form must be completed, returned to the Office Manager, and submitted to church leadership for approval.
2.Any non-member group applicant shall furnish a current Certificate of Insurance with a minimum liability limit of $1,000,000 and include The Lutheran Church of St. Mark as an additional insured.
3.A non-member applicant shall indemnify and hold harmless The Lutheran Church of St. Mark (see application form for wording).
4.Please report all damage and problems to the Church Office. The applicant agrees to pay for any damage occurring through their use of the facilities.
5.Applicant may have access to only those areas and times requested on the application and approved. Arrangements for other access can be made through the church office.
6.When facility use is concluded the leader/responsible party will see that all windows are closed, lights are turned off, all doors are locked, and the building is otherwise secure.
7.Smoking and alcoholic beverages are not permitted in the building or on the grounds.
8.Additional specific rules for use of the Parish Hall and kitchenare posted in those areas.
9.The Lutheran Church of St. Mark may preempt a scheduled event for a church-related function. If possible, a thirty-day notice will be given.
10.There may be a security deposit required, depending on the type of event and frequency of use. The deposit will be refunded upon compliancewith the General Rules for Use and the return of any keys issued for facility access.
