WashingtonCounty Juniors Participation Agreement
SectionI. Condition of Participation
The undersigned player and parent/guardian, hereafter designated as member, agrees to accept membership in Washington County Juniors (WCJ) for the 2017-2018 season.
*Participation entitles the member to participate in practices and tournaments designated by WCJ. Equal opportunity to participate will be provided during practices, however, on-court time at tournaments may be limited and will be determined by the coach and club staff.
*Uniforms will not be distributed until payment is received and participation in tournaments will not be allowed if payment is past due.
*As parents, we understand the time commitment involved and the importance of participation in all practice and tournaments. We understand the financial obligations we are making to WCJ and the coaching staff and that all fees must be paid in full even if my child no longer participates.
*Understanding the stipulations and having discussed these with our child, we agree to and will support her participation in Washington County Juniors. We understand that once registered with Washington County Juniors, she will be unable to transfer clubs in the 2017-2018 volleyball season (per USAV rules).

*As a player, I understand the commitment required for participation with Washington County Juniors and am willing to commit the time and effort to the program and my teammates for the 2017-2018 season. I understand that all decisions made by the Washington County Juniors coaching staff and administration are made in the best interest of my teammates and me, and I will abide by such decisions in a positive manner. Section II. Club Fees
We, the parents/guardians of ______have reviewed the information concerning the policies and practices of Washington County Juniors.(WCJ ) We accept the invitation to join WCJ for the 2017-2018 season and agree to pay the club fees as outlined in the Washington County Juniors fees table. We understand that any payment not received by WCJ before OR on the due date will result in a $25.00 late fee. We agree to pay a $25.00 service charge for all returned checks. We understand that we are responsible for the fees indicated for our player as well as her transportation to all practices and tournaments. I further understand that all fees will be paid in full regardless of the duration of participation, whether by injury, voluntary withdrawal or by suspension or expulsion for violation of club guidelines or regulations. This is due to the fact that membership in Washington County Juniors is limited, costs are calculated on full season participation, and the club makes non-refundable financial commitments.

III. Conclusion
In the event of default, and this obligation is referred to an attorney, and/or a collection agency, the member agrees to pay, over and above their liabilities; such as, reasonable club attorney fees, court costs, and cost of collection. By use of the facilities provided by Washington County Juniors, the member expressly agrees that WCJ shall not be liable for any damages arising from personal injuries sustained by the member or her guests in, on, or about the premised of said facilities; and further agrees that the club shall not be liable for loss or theft of personal property. Members assume full responsibility for any injuries and damages or losses, which may occur to the member or guest, in or about the premises of said facilities do hereby fully and forever release and discharge Washington County Juniors directors, officers, employees, and agent from any and all claims, demands, damages, rights of action, or cause of action, present or arising out of the member’s or guest’s use of or intended use of the said facilities or the facilities and equipment thereof provided. The undersigned understands and agrees to abide by and cause family members and guests to abide by the rules and regulations of the club as they now exist and as they may from time to time be amended or supplemented.
