Note: All referral for any initial evaluation should be coordinated first thru Students Services Office. / Date / Initials
1. / Evaluation Planning Meeting
a. Review information from the Referral Packet that you will receive from SPED Office. Contact School Psychologist and/or Speech Pathologist to schedule a pre-evaluation planning meeting.
b. Complete Referral Form (GV).
c. Send a Notice of Meeting (GV) to invite parents and other team members to the evaluation planning meeting. (Allow a 10-day notice or a date agreed upon by parent/s).
d. Meet to discuss the need for formal assessments.
e. Give parents Notice of Procedural Safeguards and obtain parent signature on Procedural Safeguards Received Form
f. Obtain signed Parent Consent to Evaluate (GV) and scan copy to the Admin. Asst. in SPED Office. (Upon obtaining parent consent, be sure to update and finalized Consent Status in GV).
g. If medical records are needed please contact the Admin. Asst. in SPED Office. A release of information will be generated.
h. After receiving signed consent to evaluate & PRIOR to conducting any evaluation(s), prepare and send a Written Notice (GV) to notify parents of the district’s intent to evaluate with a summary of the evaluation plan. Document Procedural Safeguards were provided.
2. / Evaluation
a. Conduct needed evaluations, Classroom Observation (if a learning disability is suspected), and collaborate with related service providers to transfer evaluation data to DRAFT ESER (GV).
3. / Eligibility/IEP Meeting
a. Coordinate with parents, student (if appropriate), classroom teacher, administrator, and related service providers to determine meeting date. (refer to IEP Team Configuration requirement of the SPED Handbook pg.47)
b. Send a Notice of Meeting (GV) (allow a 10-day notice or a date agreed upon by parent/s) to parents and other team members, and indicate intent to review evaluation results, determine eligibility and develop an IEP (if eligible).
c. Hold meeting to determine eligibility. Provide parents with a copy of all evaluations. Update, complete meeting notes and finalize the ESER (GV) as per meeting
d. Complete the signature pages of the ESER and document distance participation of any team member in the signature page.
e. Complete a 90-day Limit Report (Only if beyond 90-day limit)
f. If ineligible initiate a draft Written Notice (GV) indicating intent to implement eligibility decision and state parent was given copy of all evaluations. Proceed to instruction #4
g. Complete the signature pages of the IEP and BIP if any, and document distance participation of any team member in the signature page.
h. If eligible, complete an IEP (GV) & Behavior Intervention Plan (GV) if needed.
i. Complete WIDA/ACCESS Form if student is identified as ELL.
j. If appropriate, the IEP team must consider whether or not the student meets the criteria for the Alternate Assessment. Use the statewide assessment guidelines.
k. If student is under 5 yrs/6 mos. old, e-mail School Psych &/or SLP (copy to SPED Admin. Asst.) so they can complete Child Outcomes Summary Form.
l. Obtain signed Parent Consent for Initial Placement (GV).
4 / Scan all signature pages underlined forms to SPED Admin. Asst. within 3 days after the meeting, and send Original signature pages to SPED Office Immediately via inter-office mail. You will receive an email from the SPED Admin. Asst. regarding any compliance needs/revisions.
5. / Upon receiving a “compliant” note from the SPED Director, finalize the Written Notice (GV) to the parent indicating eligibility decision (draft in 3.f), and (if eligible) indicate intent to implement the IEP and BIP if any, and that copy of the ESER, IEP (if eligible), and BIP (if any) are provided.
6. / Document and send in any changes to the teacher/aide schedules.
