ATHLETIC REGISTRATION GUIDELINES

Date: January, 2015 Season: Spring 2015

Dear Parents/Guardians and Student-Athletes:

The following information is designed to provide a smooth and efficient development of our 2015 spring sports programs. Your cooperation in working with these materials is essential. Please take your time and fill out all forms completely.

The following information can be found by completing the following steps:

·  You will find the following information on this page

The following forms must be downloaded signed and turned in together.

1. Glen Ridge High School Athletic/Band Training Rules: This form must be downloaded and completed by the student-athlete and parent/guardians.

2.  Board of Education Policy 9162: Athletic Code of Conduct: This form must be downloaded and must be read and SIGNED by parents/guardians and athletes EACH TIME the student-athlete participates in a sport.

3.  Sudden Cardiac Death in Young Athletes Acknowledgement Form: This form must be downloaded and SIGNED by parents/guardians and athletes EACH TIME the student-athlete participates in a sport. The brochure is located on the Athletic Forms page just above the acknowledgement form.

4.  The NJ Department of Education Sports-Related Concussion and Head Injury Fact Sheet and Parent/Guardian Acknowledgment Form: Must Be Read signed by Parent/Guardian and student.

5.  ImPACT Letter and permission slip: Must be signed by student and parent/guardian.

6.  NJSIAA Steroid Testing Policy: NJSIAA State form must be signed by BOTH Student-Athlete and parent/guardian (and dated). EACH TIME the student-athlete participates in a sport.

7.  Health History Update Questionnaire: This form must be completed EACH SEASON the student-athlete participates in a sport. The health history update questionnaire addresses the specific questions recommended by the American Heart Association.

8.  Pre-Participation Physical Examination - History Form: A physical is good for 365 days; please check the date of your child’s last physical. If your child’s physical has expired, they will need to have a physical completed, signed by your physician and the school district physician prior to participation. All physicals must be completed on the new NJ DOE pre-participation physical evaluation forms.

Clearance Form

If anything is incomplete the forms shall be returned to the student’s medical home for completion.

If you child has had a physical within the last 365 days and if it is on file in the school nurses’ office, you will not need a physical until the original physical expires. A Pre-Participation Physical Examination may be needed only ONCE during the school year IF your child’s athletic season falls within the 365 days.

ATHLETIC REGISTRATION FORMS ARE DUE TO THE COACHES MAILBOX IN THE MAIN OFFICE BY FEBRUARY 23, 2015

ANY FORMS TURNED IN AFTER THIS DATE WILL BE CONSIDERED LATE AND STUDENT-ATHLETES MIGHT NOT BE ABLE TO PARTICIPATE BY THE START OF THE FIRST PRACTICE.
