Holiday Service
WHAT IS THE PURPOSE OF THIS SERVICE?
This service offers Holiday (Christmas) incentives for Children and Elders as well as a Community meal.
PWNA does not guarantee that a Program Partner will be approvedfor the Holiday (Christmas) service bysimply submitting a request form. All requests are reviewed and each Program Partner will be notified of your approval or denial after the deadline.

PARTNER EXPECTATIONS
- Be an established organization that provides services to either Children or Elders in the community
- Require participants to sign the distribution log (sign out sheet) for incentives received
- Report to PWNA accurately and on time
- All distributions should occur within a reasonable amount of time, prior to the end of the year
- Provide adequate and secure storage for the incentive items
MINIMUM REQUIREMENTS
- Must submit the request by the deadline of October 15th
- Must be able to provide an event date for your distribution or meal and update the PWNA office of any date changes that occur
- Provide volunteers/staff to assist with the delivery and the distribution
REPORTING
A one-page report will be required 30 days after theHoliday (Christmas). You must include with your Report:
- Sign out sheets with the names of the recipients and their signatures
- If pictures (please include a photo release form) or interesting stories are taken please forward them with your Report.
