REQUEST TO HOSTAN EMS ASSESSOR TRAINING WORKSHOP

This service is subsidised by the Ministry of Health (MOH) as part of the Learning and Development Programmes for the Wheeled Mobility & Postural Management Level 1 and Level 2 credential. Only services with a MOH DSS contract may request an EMS assessor training workshop. As of October 2016, the MOH are no longer subsidising Refresher workshops. Seating To Go will schedule these periodically based on demand.

Costs include gst and are for up to 20 attendees.

The total cost is for up to 20 people and will be invoiced to your service2 weeks prior to the workshop unless otherwise agreed. Any part charging for therapists from outside your service that you have agreed may attend is your responsibility to invoice – see following page for access and costs for MOH subsidised EMS assessor training andmaximum charge out rates.
Generally only assessors who meet the MOH training access criteria may attend. If there are other assessors or suppliers wanting to attend, please contact Seating To Go to discuss.
A minimum of 10 attendees is required for the provision of this workshop.
Additional attendees may be considered but this must be discussed with Seating To Go in the first instance and will incur an additional charge per person.
The contact person’s details will be available on the Seating To Go website for therapists wanting to register for this workshop. Your service is responsible for administering registrations.
Your service is responsible for advising Seating To Go when the workshop is full so that details can be amended on the website.
Your service is responsible for providing the venue and morning & afternoon teas.
Your service will be sent an equipment list prior to the workshop to request equipment direct from suppliers. Your service will arrange and fund the return of any equipment used in the workshop to suppliers.
Seating To Go will provide all workshop materials and handouts, certificates of attendance, travel and accommodation for facilitators.

ACCESS AND COSTS FOR EMS ASSESSOR TRAINING

IF YOUR SERVICE IS HOSTING A MOH SUBSIDISED WORKSHOP, IT IS AGREED WITH MOH THAT THE FOLLOWING COSTS CAN BE ON-CHARGED TO ATTENDEES FROM OUTSIDE YOUR SERVICE.
