Release and Hold Harmless Agreement

This RELEASE AND HOLD HARMLESS AGREEMENT is made and entered into on this

day of , 2012, by , a student in

the AviationCareersAcademy (hereinafter “Student”), and , the parent and/or legal guardian of said student (hereinafter “Guardian”).

WHEREAS Tulsa Technology Center and Tulsa Community College are members of the Aviation Education Alliance, which sponsors and conducts the Aviation Careers Academy, enrollment in which is available to students in the 10th, 11th, and 12th grades to provide them an opportunity to explore aviation career possibilities.

For and in consideration of good and valuable consideration, the receipt and sufficiency of which are hereby acknowledged, student and Guardian, for themselves individually and for their personal representatives, heirs, executors and assigns, do hereby promise, covenant and agree to release, hold harmless and indemnify Tulsa Community College, Tulsa Technology Center and any outside facilities toured, their regents, trustees, officers, employees, agents, and representatives, from and against any and all liability, loss, damages, claims, demands, actions or rights of action of whatever kind or nature, either in law or equity, arising from or by reason of any personal injury, death, or property damage resulting or in any way connected with Student’s participation in the Aviation Careers Academy.

The undersigned recognize, acknowledge, and hereby assume the risk and danger involved in Student’s participation in the AviationCareersAcademy and its activities, which include but are not limited to flight instruction and approximately one to three hours of air flight in aircraft provided by the AllianceFlightCenter. The undersigned hereby expressly acknowledge and assume the risk of potential personal injuries, both known and unknown, death, and/or property damages which could occur in connection with air flight and Student’s participation in the Academy.

The undersigned hereby state that they have each carefully read the foregoing RELEASE AND HOLD HARMELSS AGREEMENT and know the contents hereof and sign this document as their own independent free and voluntary act. Guardian herby certifies the h/she is the Parent and/or Legal Guardian of Student and herby gives consent and authorizes the AviationCareersAcademy, the members of the Aviation Education Alliance, and their agents, employees and representatives to secure emergency medical treatment for sad Student while he/she is in attendance at said Academy.
